Performance Work Statement (PWS)

To provide Continued Readiness Program (CRP) and Life Safety Assessment services for the Environment of Care (EOC) Program at Naval Hospital Jacksonville (NHJAX) to ensure compliance with Joint Commission and NFPA 101 Life Safety Code standards.

• Conduct four (4) scheduled on-site visits annually, each lasting four (4) days, for a total of sixteen (16) on-site days per year.

• Perform comprehensive audits and inspections of EOC components: Safety, Security, Fire Safety, Hazardous Materials and Waste, Medical Equipment, Utilities Management, and Emergency Management.

• Verify compliance of records and documents, including Management Plans, Risk Assessments, audits, annual evaluations, and committee activities.

• Conduct physical site evaluations, hazard surveillance tours, and random staff challenges.

• Perform life safety walk-throughs to assess physical readiness (e.g., smoke barriers, rated walls).

• Develop and update Life Safety Drawings twice annually for Building 2080 (hospital) and Building 2034 (substance abuse rehab facility).

• Complete a Comprehensive Statement of Conditions assessment annually (unless The Joint Commission visits) to certify compliance with NFPA 101 standards.

• Develop a Plan for Improvement (PFI) detailing deficiencies, proposed actions, timelines, and estimated costs.

• Provide telephone support during Joint Commission inspections.

• Written reports submitted within 14 calendar days after each site visit.

• Updated Life Safety Drawings provided twice per year.

• Comprehensive Statement of Conditions document completed annually.

• Plan for Improvement documentation submitted electronically.

• Contractor's assessment team must have a majority with at least four (4) years of experience with TJC standards and the Life Safety Code.

• Base period of twelve (12) months with four (4) 12-month option periods, plus a potential six (6) month extension.

• Place of performance: Naval Hospital Jacksonville, Building 2080 Child St., Jacksonville, FL 32214.

• Work hours: Monday through Friday, 0700 to 1600 hours, excluding Federal holidays.

• Contractor must develop and maintain an effective Quality Control (QC) program.

• Contractor personnel require base access coordination and must adhere to all base regulations and security protocols.

• Compliance with DoD cybersecurity and information assurance requirements (DoDI 8510.01, NIST SP 800-53, etc.) is mandatory.

• Contractor personnel must clearly identify themselves as contractor support staff in all communications.
