The Phoenix Area Indian Health Service (IHS) is gathering information to determine the capabilities of various business concerns (Indian Small Business Economic Enterprises, Other Small Business Concerns, and Other than Small Business Concerns) to provide a specific brand-name product and associated services.

• Requirement: The government is seeking a three-year Firm-Fixed-Price contract for Core Strengths / Strength Deployment Inventory (SDI) 2.0 platform subscription, assessment credits, and support services from the Original Equipment Manufacturer (OEM), Crucial Learning.
• Key Components: The contract would include SDI 2.0 assessment credits, annual subscription access to the Core Strengths platform (for up to 2,500 users), reporting, team analytics, data export, implementation, and customer support services.
• Brand-Name Product: The specific product is Core Strengths / Strength Deployment Inventory (SDI) 2.0 by Crucial Learning. Vendors who are not the OEM must provide proof of authorization from Crucial Learning.
• Technical Capabilities: Bidders must demonstrate capability in platform licensing (up to 2,500 users, annual credits), functional capabilities (assessment administration, reporting, portability, analytics, data export), and implementation/support.
• Experience: Experience with SDI 2.0 or equivalent OEM-authorized solutions, enterprise-wide implementations, and Federal/healthcare organizations is desired.

• Buy Indian Act: Priority will be given to Indian Small Business Economic Enterprises (ISBEE). Respondents asserting ISBEE status must provide specific ownership, control, and performance details.
• Submission Instructions: Responses are limited to 7 pages and must include company information, SAM UEI, business size, capability statement (addressing Sections 3 & 4), proof of authorization (if not OEM), FSS Contract Number (if applicable), and past performance. Responses are due by the date stated in SAM.gov.

Statement of Work (SOW)

To provide continued access to the SDI 2.0/Core Strengths platform to support relationship awareness, team effectiveness, leadership development, and conflict resolution for the Phoenix Area Indian Health Service (PAIHS).

• Provide a three-year subscription/licensure for the SDI 2.0 platform.

• Include annual assessment credits (878 for Base and Option Year 1, 877 for Option Year 2).

• Provide annual subscription access to the SDI/Core Strengths platform for up to 2,500 members.

• Offer access to SDI-related tools, user guides, and training-support materials.

• Enable electronic administration of assessments, reporting, and portability of individual results.

• Facilitate consolidated analysis across multiple PAIHS locations.

• Base Period: 12 months from date of award.

• Option Period 1: 12 months.

• Option Period 2: 12 months.

• Firm-Fixed-Price (FFP) contract.

• Online assessments accessible via modern web browsers.

• Role-based administrative access.

• Data privacy and handling protocols to be followed.

• Compliance with Section 508 accessibility requirements.

• Eligibility Period: The enterprise must meet the definition throughout the offer period, at the time of contract award, and during the full contract performance period. Any change in eligibility requires immediate notification to the Contracting Officer.

  • Name of the 51% owner of the IEE Firm
  • Business Name
  • Unique Entity Identifier (UEI) Number
  • Certifying Signature and Print Name
