Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for contracted services to conduct job-related medical surveillance screening examinations and Occupational Health (OH) services for DC Army National Guard employees potentially exposed to workplace health hazards.

• Provide a qualified mobile OH unit with an affiliated fixed facility.

• Conduct medical surveillance screening examinations and OH services.

• Services include audiograms, pulmonary function tests (PFTs), vision screening, medical history reviews, screening exams, and respiratory history/clearance.

• Determine employee fitness for duty (FFD) for assigned duties without risk of harm.

• Services are for testing, screening, and evaluation; medical treatment is not authorized.

• Services must comply with relevant regulations (OSHA, CFR, Army Regulations, DoD Instructions).

• Contractor must maintain an effective Quality Control Plan (QCP).

• Performance will be evaluated per the Quality Assurance Surveillance Plan (QASP).

• Specific performance objectives and standards are detailed in Technical Exhibit 1.

• Service Delivery Dates: Options include July 22-23, July 29-30, or August 26-27.

• Service Completion: Within 30 days of the 2-day medical surveillance event, including submission of hard copy reports.

• Location: District of Columbia Army National Guard, 2001 East Capitol Street SE, Washington, DC 20003.

• Hours of Operation: 0900-1700hrs, Wednesday through Thursday, excluding US holidays.

• Contractor employees must complete AT Level 1, iWATCH, and OPSEC training.

• Adherence to installation regulations, professional conduct, and zero-tolerance policies (sexual harassment, assault, discrimination, trafficking).

• Contractor must safeguard Government property and secure facilities.

• Strict confidentiality and data protection for Protected Health Information (PHI) and Personally Identifiable Information (PII) are required.

• Contractor must designate a Contract Manager (CM) and an alternate.

• Personnel qualifications include licensed MD, PA, NP for test review, CAOHC certification for audiograms, and NIOSH certification for PFTs.

• Government will provide workspace, office items, and utilities. Contractor furnishes all other equipment, labor, and services.

This solicitation is for the DC Army National Guard Occupational Health (OH) unit to procure contracted services for a qualified mobile OH unit with an affiliated fixed facility. The services required include conducting job-related medical surveillance screening examinations and OH services for employees who may be exposed to health hazards in the work environment.
