Sources Sought / Market Research

This document is a Sources Sought notice issued by the Indian Health Service (IHS) on behalf of the Phoenix Indian Medical Center. The purpose is to conduct market research to identify potential contractors capable of providing "5 – ADEC – Dental chairs and accessories." This is not a Request for Proposal (RFP).

• The procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items.

• Vendors must be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller.

• OEM warranty and service must be provided and maintained by the OEM.

• Quotes/responses must include a copy of the authorized distributor letter from the manufacturer.

• Quotes/responses must identify the vendor as an Indian Small Business Economic Enterprise (ISBEE) / Native American-owned small business; otherwise, the offer will not be considered.

• The requirement will not be split; quotes must include all aspects of the RFQ (all equipment, services, and options).

• Place of Performance/delivery/service: Phoenix Indian Medical Center, 4212 N. 16th Street, BLDG 9, Phoenix, AZ 85016.

• Payment will be made through IPP (Invoicing Processing Platform).

• HHSAR 352.232-71 Electronic Submission of Invoice Payment Requests will apply.

• Responses should identify socio-economic status in accordance with SBA size standard.

• Deadline for response: No later than 5:00 PM EST on April 29, 2026.

• This is a Sources Sought specifically for ISBEE – Indian Small Business Economic Enterprise Set-aside.

• Responses must identify the vendor as an Indian Small Business Economic Enterprise/Native American-owned small business.

• The IHS is using this tool to assist in developing a possible RFQ/RFP and determining if a small business set-aside is possible or if full and open competitive procedures should be utilized.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide comprehensive on-site services for the de-installation of existing A-dec dental equipment and the procurement, delivery, assembly, installation, and inspection of new A-dec dental equipment at the Phoenix Indian Medical Center (PIMC).

• De-install and safely remove all existing A-dec dental equipment from the PIMC Dental Clinic, including proper disposal or recycling of components.

• Procure, deliver, assemble, install, and certify new A-dec dental equipment, ensuring compliance with all OEM installation criteria and technical standards.

• Install new equipment without modifications to existing structure or utilities.

• Provide comprehensive on-site visits for installation, configuration, and training.

• Perform inspection and performance testing of all newly installed A-dec dental equipment.

• Provide orientation and end-user training to PIMC dental department staff.

• Provide a detailed service report upon completion of services.

• Procurement and installation of 5 x A-dec 511 Dental Chairs.

• Procurement and installation of 5 x A-dec 541 - 541 12 O'clock Duo INSTM Delivery Systems.

• De-installation services for existing A-dec dental equipment.

• Delivery and blanket-wrap services for new A-dec equipment.

• Inspection and performance testing of new equipment.

• Post-installation training for dental staff.

• A minimum 5-year warranty for parts and labor on all A-dec equipment and components.

• The period of performance includes a base period plus 4 option years from the issuance of award.

• Services are to be performed at Phoenix Indian Medical Center, 4212 N 16th St, Phoenix, AZ 85016.

• Services are to be performed during normal operating hours (Monday-Friday, 8:00 a.m. to 4:30 p.m.), unless otherwise specified and arranged.

• Contractor must be an A-dec certified installer and authorized dealer.

• All equipment and accessories must be new, A-dec factory assembled/configured, and include a factory warranty.

• Contractor must provide documentation of A-dec certification and OEM authorization.

• Installation must comply with various standards including OEM, ANSI/ADA, NFPA, TJC, and OSHA.

• Contractor must provide a projected schedule within 15 days of contract award.

• Contractor must provide at least two site visits prior to installation and one for final review/testing.

• Contractor must warrant all workmanship and provide a minimum 5-year warranty for parts and labor.
