Statement of Work (SOW) / Performance Work Statement (PWS)

To procure comprehensive dental lab services for eligible dental patients of the Ralph H. Johnson Veterans Affairs Medical Center (VAMC) and its outpatient clinic.

• Provide 24/7 dental lab services, including holiday coverage.

• Offer same-day turnaround for specific services and defined turnaround times for others (24 hours, 48 hours, 3 working days, 5 working days, less than two weeks, less than three weeks, less than four weeks).

• Services include fabrication and repair of various dental prosthetics such as complete dentures, removable partial dentures, crowns, bridgework, surgical templates, implant-supported prostheses, and radiographic guides.

• Utilize specified materials: Lucitone 199 high impact acrylic, Dental Diamond D Acrylic, Injectable Ivoclar, and Trubyte TMIPN for tooth material.

• Include digital dentistry services.

• Provide daily pickup and delivery services Monday through Friday.

• Furnish all labor, transportation, materials, and equipment.

• Adherence to specified turnaround times for different types of dental prosthetic fabrication and repair.

• Quality Assurance Surveillance Plan (QASP) will be used to monitor contractor performance.

• Performance standards will be defined at contract award.

• Failure to meet performance measures may result in a 25% reduction in monthly payment.

• Billing must be submitted monthly and within 6 months of service.

• Base Year (to be determined at award) with 4 subsequent one-year option periods.

• Services to be performed at the Ralph H. Johnson VAMC, North Charleston Outpatient Clinic, Charleston, SC.

• Contractor personnel must be licensed and qualifications subject to VAMC review and approval.

• Contractor facilities are subject to inspection and must hold all required local and state licenses and permits.

• Use of specific materials (Lucitone 199, Dental Diamond D Acrylic, Injectable Ivoclar, Trubyte TMIPN) is required.

• A Quality Assurance Surveillance Plan (QASP) will govern performance monitoring.
