• Vegas - Office Fees Sheet: Provides fixed prices per treatment for dental services in the Las Vegas area, along with an estimated 43 soldiers.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide dental treatment services for the Nevada Army National Guard (NVARNG) to ensure Soldiers achieve or maintain a Dental Readiness Classification (DRC) of 1 or 2, thereby supporting Individual Medical Readiness (IMR) and overall force readiness.

• Provide all personnel, facilities, equipment, and materials for dental treatment services.

• Services are limited to procedures necessary to correct dental conditions identified during NVARNG dental readiness examinations.

• Focus on treating approximately 130 Soldiers identified as Dental Class 3 to restore deployable dental status.

• Services include diagnostic, restorative, endodontic, periodontal, prosthodontic, and oral surgery procedures as authorized.

• Higher cost procedures (e.g., crowns, root canals, dentures, surgical treatments) or those exceeding $500 require prior approval.

• Prioritize treatment for Soldiers preparing for mobilization or deployment.

• 100% compliance with approved treatment plans.

• Documentation submission within three (3) business days of treatment completion.

• Bi-weekly reports required, including Soldier details, procedures, medications, and final dental readiness status.

• Invoicing within ten (10) calendar days of services rendered via Wide Area Workflow (WAWF).

• Period of Performance (POP): Estimated from July 15, 2026, to July 14, 2027.

• Place of Performance: Contractor-operated dental clinics within a fifty (50) mile radius of Carson City, Nevada, and Las Vegas, Nevada.

• Contractor personnel must possess valid Nevada dental licenses.

• Compliance with HIPAA, OSHA, ADA Standards, and Nevada Dental Board Regulations is mandatory.

• Development and maintenance of a Quality Control Plan (QCP).

• Services must adhere to applicable Army Regulations and professional dental standards.

• Use of American Dental Association (ADA) Current Dental Terminology (CDT) procedure codes for billing, referencing Appendix B for the Dental Procedure Price Schedule.
