Statement of Work (SOW) / Performance Work Statement (PWS)

To provide diagnostic reference laboratory testing services for the Phoenix Indian Medical Center (PIMC) to aid in the development of patient-specific medical management plans, reduce the risk of cancer or infectious diseases, and improve patient outcomes.

• Provide non-emergent cancer or infectious diseases testing as ordered by PIMC.

• Utilize state-of-the-art diagnostic laboratory facilities for comprehensive services.

• Furnish all necessary personnel, facilities, and equipment.

• Perform all services off-site at the contractor's facility.

• Conduct diagnostic testing for screening and detection of cancer or infectious diseases.

• Provide critical information to clinical providers and patients.

• Offer cellular analysis, protein identification (Immunohistochemistry), DNA/RNA analysis (PCR, in-situ hybridization), and Flow Cytometry.

• Provide a list/catalog of tests, procedures, and costs, including specimen requirements and handling instructions.

• Maintain a turnaround time schedule, meeting 90% of targets.

• Communicate "Critical Value" or significant abnormal test results immediately.

• Provide urgent reports within one day of specimen receipt for life-threatening conditions.

• Return test results within 7-10 working days for 90% of cases.

• Maintain a permanent register of patient data and results, with a retrieval system available within one day.

• Disclose test results via mail, telephone, fax, or secured internet access within 2-4 hours.

• Supply specimen containers, collection materials, and transport materials within 72 hours of request.

• Ensure specimens are in transport for no more than 12 hours.

• Maintain optimal specimen conditions during transport.

• Complete and report testing within 7-10 working days upon receipt of specimens (90% of the time).

• Routine Tests: 85% completed within 3 working days, rest up to 7 days.

• Cytology Tests: 95% completed within 5 working days.

• Histopathology tests: 95% completed within 14 days.

• Less than 3 unfavorable incidences per quarter for all PAIHS sites related to specimen transport and integrity.

• Base plus four (4) one-year option years, commencing from the date of award.

• Services are performed off-site at the contractor's facility.

• Contractor must be CLIA Licensed and Laboratory Accreditation Certified.

• Specific instructions for specimen collection, labeling, and shipping (FedEx/UPS).

• All-inclusive unit price covering travel, lodging, per diem, fringe benefits, taxes, etc.

• Invoices must comply with the prompt payment act and be submitted electronically.

• Designated Contracting Officer Representative (COR) will monitor performance.

• Government reserves the right to accept or reject services based on quality.

Solicitation / RFP / RFQ

This is a Request for Proposal (RFP) for diagnostic reference laboratory testing services on an "As Needed Basis" for the Phoenix Indian Medical Center in Phoenix, AZ. The contract will be for a period of five years.

• Diagnostic reference laboratory testing services.

• Services are to be provided on an "As Needed Basis."

• Contract Type: Anticipates awarding a single or multiple Blanket Purchase Agreement (BPA) Firm Fixed-Priced Contract.

• Period of Performance: Initial period is June 1, 2026, through May 31, 2027, with four (4) 12-month option periods.

  • Option Year 1: June 1, 2027 - May 31, 2028
  • Option Year 2: June 1, 2028 - May 31, 2029
  • Option Year 3: June 1, 2029 - May 31, 2030
  • Option Year 4: June 1, 2030 - May 31, 2031

• All-inclusive pricing is defined as all costs/expenses to be included, such as travel, labor, supplies, materials, Federal/Local/State tax, and overhead.

• Proposals are to be submitted electronically in PDF format.

• Proposals must be submitted by email to the Contract Specialist (dale.clark@ihs.gov) by May 5, 2026, at 1:00 PM MST.

• The Technical Proposal shall not exceed 10 pages and must address: Technical Approach, Staffing Plan, Quality Control, and Safety and Infection Control Compliance.

• Offerors must provide a minimum of two (2) and a maximum of five (5) recent and relevant past performance references.

• Price Proposals must complete the CLIN pricing schedule, including all labor, materials, equipment, overhead, profit, and disposal costs.

• Evaluation Factors: Technical Capability and Past Performance (combined) are significantly more important than Price. Price will become more important as technical proposals become more equal.

• Offers must be firm for at least 90 days from the date of receipt.
