• Corrective Maintenance (CM): Assess, inspect, test, and troubleshoot CM service requests within five business days of notification. Provide written estimates within two business days of assessment. Repairs must be completed within five business days of receiving all necessary parts, unless otherwise approved. Includes functional tests (load tests) and proper lug nut torquing.

• General Services: Provide all personnel, equipment, tools, supervision, and quality control. Use lock-out/tag-out procedures. Drain and purge operations require secondary containment and appropriate PPE. Hazardous materials/waste must be transferred to approved containers. Contractor is responsible for removing replaced parts and trash.

• Acceptable Performance Levels (APLs): Specific APLs are defined for various deliverables including signing in on the visitor log, timely and complete PMs, affixed PM stickers, written filter dates, maintenance sheets, corrective maintenance estimates, timely and complete CM, and call-back service. Failure to meet APLs may result in payment withholding.

• Period of Performance: One 12-month base period and two 12-month option periods, not to exceed three years total.

• Place of Performance: DLA DS Lewis facilities, to the maximum extent possible, during normal business hours, excluding recognized US federal holidays or installation closures.

• Contract Type: Firm Fixed Price (FFP) time and materials contract.

• Federal Holidays: Contractor is not required to provide services on 11 Federal Holidays unless approved.

• Base Access & Security: Contractor must coordinate base access for employees and comply with all Base Command policies, including security regulations and identification requirements.

• USG Furnished Property/Materials/Equipment/Services (GFP/M/E/S): USG will provide workspace and utilities. USG will not provide materials. USG may provide MHE at its discretion for PM/CM assistance.

• Contractor Furnished Property/Materials/Equipment/Services (CFP/M/E/S): Contractor must provide all materials for PM/CM and spill cleanup, PPE, contact information, and Certified Forklift Technician (CFT) certifications.

• Quality Assurance (QA): Contractor must develop and maintain a Quality Control Plan (QCP).

• Spill Responsibility: Contractor is solely responsible for all spills/leaks and their cleanup, complying with applicable laws and regulations. Reports are required immediately and in writing.

• Invoicing: Monthly invoicing through DoD Wide Area Workflow (WAWF).

• Contract Minimum/Maximum: Minimum $2,500; Maximum 150% of the estimated contract value. No minimum for option periods.
