This solicitation is for the replacement of two dock lifts at Building 1 of the Veteran Healthcare System of the Ozarks (VHSO) in Fayetteville, AR. The existing lifts are beyond normal maintenance and require replacement to ensure continued delivery of mission-critical supplies.

• Removal and legal disposal of existing dock lifts and associated utilities.

• Site preparation for new installations.

• Procurement and installation of new dock lifts and site/utility improvements.

• Startup, testing, and training for government staff on new lift operation and maintenance.

• All waste removal and legal disposal.

• Work must be performed in a workmanlike manner, code-compliant, and fully functional.

• Demolition and installation activities must minimize impact on daily facility operations.

• Contractor must provide all labor, equipment, and materials.

• Contract type: Firm Fixed Price (FFP).

• Period of Performance: 90 days after written notice to proceed.

Statement of Work (SOW) / Performance Work Statement (PWS)

To remove and replace two existing dock lifts at the Back Dock Building 1, Veteran Healthcare System of the Ozarks (VHSO) in Fayetteville, AR. These lifts are critical for the delivery of mission-critical supplies.

• Removal and legal disposal of existing dock lifts and associated utilities.

• Demolition, debris removal, and site preparation for new installations.

• Procurement and installation of new dock lifts and necessary site/utility improvements.

• Procurement and installation of new dock lifts and all site/utility improvements as identified in the Statement of Work and in accordance with manufacturer's instructions.

• Contractor to provide startup, testing, and training of Government staff on new lift operation and maintenance.

• Work must be performed in a neat, workmanlike, and code-compliant manner.

• Contractor is responsible for all project waste disposal.

• Demolition and installation activities must be sequenced to minimize impact on facility operations.

• Contractor must coordinate with EPM/COR and shipping/receiving staff for logistics planning.

• Shutdowns of electrical/water must be coordinated with the facility.

• Contractor must provide all labor, equipment, and materials.

• New equipment must be in place and functional to the greatest extent possible.

• Plumbing installations must be in place to the greatest extent possible.

• Work must be left broom-clean at the end of each workday.

• Daily Construction Reports are required after award.

• Period of Performance: Ninety (90) days after written notice to proceed.

• Contractor must comply with this SOW and FAR Solicitation and Contract clauses.

• Contractor must coordinate with VA Police and COR for security management.

• Contractor must wear a visible VHSO issued identity badge.

• Contractor must develop a phasing and Infection Control Risk Assessment (ICRA).

• Contractor must comply with all applicable records management laws and regulations.

• Contractor must not leave unattended out-of-service utilities, fire protection, life safety systems, or security systems without written permission.

• Contractor must identify and report any discrepancies between the SOW and field-verified conditions.

• Contractor is strongly encouraged to visit the site.

• Government may provide a forklift and operator, subject to contractor request and coordination.

• Contractor is liable for damage to government-furnished equipment or injury to the operator due to negligence.

• Minimum Qualities and Characteristics for dock lifts include: Kelly Model KDL-7295-15K-Pit Mounted or Equal, 18" standard solid or split bridge plate, dual hydraulic lifting cylinders, dual safety chains, velocity fuses, manual lowering valve, accordion bellow skirting, spring-assisted bridge, electric toe-guards, maintenance safety strut, removable handrails, pre-wired Nema 4x push button control, 15,000lbs Lifting Capacity, and removable guard rails.

• The new installation must be complete and properly functioning by the final completion date.

• Work must be performed in accordance with this SOW, manufacturer's instructions, and applicable codes and regulations.

• Period of Performance: Ninety (90) days after written notice to proceed.

• Contractor must coordinate with EPM/COR and shipping/receiving staff for logistics planning.

• Shutdowns of electrical/water must be coordinated with the facility.

• Government may provide a forklift and operator with advance notice (4-5 business days).

• Contractor is strongly encouraged to visit the site prior to submitting a proposal.

• Contractor must confirm existing conditions and dimensions before proceeding and report any discrepancies.

• Safety is the highest priority, requiring compliance with Federal, State, and Local OSHA codes, NFPA 70E, and other safety requirements, including PPE.

• Contractor must develop a phasing and Infection Control Risk Assessment (ICRA).

• Basis-of-design products are indicated, but contractor-proposed equals will be considered if they meet quality and salient characteristics.

• Submittals must include equipment details, warranty, quantity, subcontracting plan (including SDVOSB certification), rental equipment, labor, and a detailed projected schedule (Gantt chart).

• Contractor must coordinate all work with the COR and ensure minimal impact on facility operations and air quality.

• Daily construction reports, material location reports, and unusual event reports are required.

• Strict adherence to records management policies (Federal Records Act, NARA, FOIA, Privacy Act) is mandatory.

• Site supervision requires qualified personnel, adherence to security protocols, visible ID badges, and proper key management. Lost badges or keys will incur fees.
