Scope of Work

Performance Standards & Deliverables

• The contractor must develop and maintain an effective Quality Control (QC) Program.
• Performance will be evaluated by the Government using a Quality Assurance Surveillance Plan (QASP).
• Specific tasks, performance standards (NFPA 660, Contract), acceptable quality levels (0% failure), and surveillance methods (COR) are detailed in the Performance Requirements Summary (Attachment 1).
• Key Deliverables: Certifications/qualifications of DHA personnel (after award), prior dust test reports (after award), and a written DHA Report (PDF format) within two weeks of completing the onsite visit.

Special Requirements

• Contractor must provide certifications/qualifications of the individual(s) leading the DHA upon contract award.
• A schedule for the DHA must be provided to the CCAD POC at least two weeks in advance of data collection.
• Data collection plans involving instrumentation require review at least two weeks in advance.
• Contractor personnel must comply with all security requirements, including identification and training.
• A senior employee must be designated as an on-site security point of contact.
• Applicable publications include NFPA 660 (Mandatory) and FAR Subpart 42.5 (Mandatory).
