Statement of Work (SOW) / Performance Work Statement (PWS)

To provide monthly elevator maintenance services, annual elevator certification, and infrequent emergency service orders for three (3) elevators at Yuma Proving Grounds (YPG), Arizona.

• Monthly Preventative Maintenance and Inspections: Includes cleaning, lubrication, adjustment, and minor repairs to ensure safe and efficient operation.

• Annual Certification: Performed by a certified state technician.

• Emergency Service Orders: Response required for system failures that endanger personnel, property, or disrupt operations.

• Repairs: Disassembly, repair, or replacement of defective parts, using manufacturer-specified or equivalent quality parts.

• Reporting: Monthly inspection checklists and reports, emergency repair reports, and annual certification reports are required.

• Response Times: 1-hour for trapped passengers (0600-1630 M-Th), 1.5-hour for trapped passengers (Fri-Sun/Holidays), 2-hour for other emergencies.

• Routine Service Completion: Within 3 business days of approved service order.

• Emergency Repair Completion: Within 4 hours for critical issues, timely for others.

• Reporting Submission: Within 3 business days for monthly reports, 3 days for emergency reports, 6 days for annual certification reports.

• Quality Control: Must meet manufacturer's specifications, safety regulations, and building codes. A Quality Control Plan (QCP) is required.

• Period: One (1) Base Year of 12 months and four (4) 12-month option years.

• Place: US Army Yuma Proving Ground, Yuma, Arizona.
