Statement of Work (SOW) / Performance Work Statement (PWS)

To replace ten (10) aging examination tables at the Yakama Service Unit to ensure continued delivery of safe, accessible, and quality patient care, and to maintain compliance with current accessibility standards.

• Furnish and deliver ten (10) fully assembled, functional, barrier-free examination tables with integrated patient support rails.

• Each unit must be new, unused, and in current production with full manufacturer warranty.

• Delivery includes inside delivery and placement in designated rooms, as the facility lacks a loading dock.

• Contractor is responsible for removal and disposal of all packaging materials.

• Equipment must meet or exceed manufacturer specifications.

• Units must be free from defects, damage, or cosmetic imperfections.

• All safety features must be operational.

• Acceptance testing includes functional testing of all mechanical and electrical components.

• Government will verify compliance through pre-delivery review, upon delivery inspection, and acceptance testing.

• Delivery required within sixty (60) calendar days from the date of contract award.

• Delivery Destination: Yakama Service Unit, 401 Buster Road, Toppenish, WA 98948.

• Delivery hours: Monday through Friday, 8:00 AM to 4:00 PM (excluding federal holidays).

• Examination tables must be barrier-free, equivalent to Midmark 626-002 or approved equal.

• Specific features include: low seat height (14.6-15.5 inches), minimum 650-pound weight capacity, adjustable height range, Clean Assist Roller System, Active Sensing Technology, hand/foot controls, quick exam/return to home functions, lockout capability, standard accessory receivers, tamper-resistant electrical receptacles, stirrups, pelvic tilt, and drawer heater.

• Required accessory: Patient Support Rails+.

• Upholstery: Dream 847 color (or equivalent), medical-grade vinyl, seamless construction.

• Delivery personnel must comply with facility security protocols and undergo background checks.

• Contractor must provide all equipment for inside delivery (lift gates, ramps, dollies, etc.).

• Full manufacturer warranty required for each unit.

• User manuals, operation guides, cleaning/maintenance instructions, and technical specifications sheets must be provided.

• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, Certifying Signature, UEI Number, and Print Name.
