The contractor, Exosome Diagnostics, Inc., is responsible for providing all materials, equipment, supervision, and staffing required to perform the full scope of this contract. Exosome Diagnostics is required to receive urine specimens from Charlie Norwood VA Medical Center (CNVAMC) and provide results for the ExoDx Prostate Test (EPI) on all medical specimens. CNVAMC laboratory personnel will ship Colli-Pee urine specimens daily to: Exosome Diagnostics, 266 Second Ave, Suite 200, Waltham, MA 02451. Exosome Diagnostics is required to fax all final reports to the Charlie Norwood VA Medical Center laboratory at (706)823-3965. Exosome Diagnostics must notify CNVAMC laboratory personnel if ExoDx Prostate Test results are significantly delayed due to technical and/or instrumentation related issues.
