Required submissions include the Price Proposal Form (Exhibit C), Excel Bid Form (Exhibit H), Subcontractor Annual Representations & Certifications (Exhibit F), and Proposal Certifications (Exhibit G). A small business subcontracting plan (Exhibit E) is required for large businesses if the subcontract exceeds $750,000.
