Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for providing two (2) full-time equivalent Advanced Practice Provider (APP) positions (Physician Assistants or Nurse Practitioners) to deliver outpatient primary care services at the Rosebud IHS Hospital Outpatient Department (OPD) Clinic. The goal is to improve population health outcomes, support chronic disease management, and provide culturally responsive primary care to the American Indian and Alaska Native (AI/AN) population served by the facility.

• Services: Provide comprehensive outpatient primary care services for adult and pediatric patients, including new patient intakes, routine visits, chronic disease management, preventive services, and acute outpatient care.

• Tasks: Conduct physical examinations, order and interpret diagnostic studies, diagnose and manage common acute and chronic conditions, provide patient education, order immunizations, perform minor outpatient procedures, and coordinate referrals.

• Documentation: Maintain accurate and timely documentation in the Resource and Patient Management System (RPMS) / Electronic Health Record (EHR) system, adhering to IHS standards and coding requirements. Contribute to required IHS performance measure documentation (GPRA).

• Coordination: Participate in multidisciplinary team meetings and communicate effectively with healthcare staff. Adhere to clinical protocols and credentialing requirements.

• Quality Improvement: Participate in facility quality improvement initiatives and contribute to the achievement of IHS clinical performance metrics.

• Clinical Quality: Services must align with core competencies, evidence-based guidelines, and IHS protocols. AQL: Zero substantiated patient safety events attributable to APP negligence or scope violation per quarter.

• Documentation Timeliness: 98% of encounter notes completed within 24 hours of patient visit.

• Licensure/Certification: 100% compliance at all times.

• Mandatory Training: 100% completion within required timeframes.

• GPRA Measures: Performance consistent with or exceeding IHS national GPRA targets for relevant clinical measures.

• Patient Satisfaction: No sustained pattern of negative patient feedback attributable to APP conduct.

• Period of Performance (PoP): One (1) year total, consisting of a 3-month base period and three (3) 3-month option periods.

• Place of Performance: Rosebud IHS Hospital OPD Clinic and Emergency Department, Rosebud, South Dakota.

• Hours: Generally Monday–Friday, 8:00 AM – 5:00 PM, excluding federal holidays, consistent with facility operating hours.

• Personnel Qualifications: Must possess appropriate education (accredited PA or NP program), current national certification (NCCPA for PAs; ANCC/AANPCB for NPs), unrestricted state licensure (South Dakota), and prescriptive authority. Must hold current BLS certification.

• Experience: Minimum of one year of professional APP clinical experience equivalent to GS-11 (for GS-12) or GS-12 (for GS-13) level, with preference for experience in FQHCs, IHS facilities, or underserved settings. Familiarity with AI/AN health disparities and cultural competency is strongly preferred.

• Credentialing/Privileging: Must complete IHS credentialing and privileging requirements prior to patient care.

• Background Investigation: Must undergo a suitability/background investigation.

• Health Requirements: Must meet specific health requirements including MMR, Varicella, Tdap, Hepatitis B, TB, annual Influenza, and COVID-19 vaccination.

• Reporting: Monthly Burn Reports and notifications at 60%, 80%, and 90% capacity thresholds are required.
