This document is the "Past Performance List of References" attachment (Attachment 4), which requires bidders to provide details about their past performance on relevant contracts. It is used to assess a bidder's capability and experience.

• Company Name: To be filled in by the bidder.
• Contract Number: Including task order numbers if applicable.
• Description of Work Performed: Detailed information about the work performed, including whether it was as a prime or subcontractor, FTE count, position types, and percentage of work performed.
• Contract Period of Performance (POP): The duration of the contract.
• Contracting Agency POC/Customer Name, Phone # & Email: Contact information for the agency.
• Contract Values: Total contract amount, annual amount, and POP for the annual amount.
• Performed by a Proposed Subcontractor, Affiliate, or Joint Venture: A checkbox to indicate if the work was performed in such a capacity, requiring further details on Page 2.
• Describe Function/Task/3rd Party Affect on Performance (Page 2): Details on the role of subcontractors/affiliates/joint ventures, including percentage of work, cost splitting, and type of services.

Bidders must complete this form accurately and thoroughly to demonstrate their relevant past performance. The information provided will be a key factor in the government's evaluation of the bidder's ability to successfully perform the requirements of the opportunity. Failure to provide complete information may negatively impact the evaluation.

Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the technical provisions and scope of work for replacing flooring at various facilities on Grand Forks Air Force Base. The contract is an "Indefinite Delivery/Indefinite Quantity" (IDIQ) type, with task orders to be issued over a five-year period.

• Furnishing and installation of various flooring types, including broadloom carpet, modular carpet, modular walk-off carpet, rubber tiles, rubber stair treads/nosing, luxury vinyl tile (LVT), transition strips, and resilient wall base.

• Removal of existing flooring and accessories.

• Patching and surface preparation of existing floors.

• Disposal of all scrap materials and debris at an off-base location.

• Verification of dimensions and site conditions.

• Compliance with specified standards (UFGS, CRI, manufacturer specifications).

• Potential for work in secure areas and near the airfield.

• Contractor must meet criteria set forth in Unified Facilities Guide Specifications (UFGS) and Whole Building Design Guide (WBDG).

• Response time: On-site within 24 hours for emergencies or issues.

• Quality Assurance: Government COR will evaluate performance through inspections.

• Warranty: Minimum one-year warranty on all work performed.

• Period: Five years (for task orders).

• Place: Various facilities located on Grand Forks Air Force Base, North Dakota.

• Contractor must utilize mandatory sources for carpet and adhesive materials as per the Enterprise Carpet Contract, adhering to Not to Exceed (NTE) pricing.

• Compliance with health and safety plans, security regulations, and environmental protection requirements.

• Subcontractor and personnel lists must be provided.

• A qualified supervisor must be on-site during work hours.

• Submittals for materials, shop drawings, and plans are required.

• Hazardous materials must be approved and managed according to Air Force and local regulations.

• Contractor is responsible for obtaining necessary base access passes for personnel and vehicles.

• Government will provide reasonable utilities (water, electricity) where available; contractor must provide own services if not available.

• Contractor is responsible for site security, housekeeping, and protection of government property.

• Asbestos remediation may be required per task order, following specific guidelines and submissions.

Sources Sought / Market Research

This notice is issued by the 319th Contracting Squadron, Grand Forks AFB, North Dakota, for market research purposes to identify potential sources for a Flooring Indefinite Delivery Requirement (IDR) at Grand Forks Air Force Base. This is NOT a solicitation.

The work involves providing material, equipment, and labor for maintaining, flooring/carpeting, and other related services. Specific tasks may include: Verification of Dimensions, Flooring Removal, Patch flooring, Surface Preparation, Installation, and Disposal of Debris and Furniture.

• The current incumbent is Greenstone Construction Inc.
• The applicable NAICS code is 238330 (Flooring Contractors).

Interested contractors should submit a capabilities package (max 5 pages) outlining their ability to provide the required services. This package must include:

• Minimum of three (3) past performance projects (completed in the last 5 years, valued between $20K-$100K) for flooring/carpeting installation or repair.

• All projects must have been rated at least "satisfactory."

• Three past references with current contact information and contract numbers.

• Details on any teaming or subcontracting arrangements, including anticipated percentages and the type of businesses (small/large) to be used.

• Responses are due by Friday, June 5, 2026, at 1600 CST.

• All questions and responses must be emailed to Monique Padilla (Monique.padilla.3@us.af.mil) and Asante Nieves (asante.nieves@us.af.mil).

Based on the responses, the requirement may be set aside for small businesses or procured through full and open competition. A single award may be made.

• This notice does not commit the Government to any contract.
• The Government will not pay for information requested or costs incurred in developing responses.
• Telephone inquiries will not be accepted.
• No feedback or evaluations will be provided on submissions.
