Statement of Work (SOW) / Performance Work Statement (PWS)

To procure the services of a Laboratory Director for the College of American Pathologist (CAP) Accredited Clinical Laboratory at the Federal Medical Center (FMC) Lexington, Kentucky.

• Provide on-site consulting services as a Laboratory Director, as defined by CAP.

• The Laboratory Director will not perform on-site evaluation of cytology or pathology specimens.

• Services include monthly consultations to the laboratory, approximately one to two hours per visit.

• Duties include being accessible for consultations, ensuring appropriate educational activities, sufficient qualified personnel, a safe environment, reviewing policies and procedures, validating new tests/instruments, overseeing the Quality Management Plan, reviewing proficiency testing and Quality Control Data, coordinating problem resolution, overseeing inspections, reviewing self-inspections, reviewing CAP accreditation items, and reviewing potential problem areas.

• The contractor must maintain medical liability insurance.

• The contractor must complete and maintain the Session-Based Invoice Verification (SBIV) for each visit, including contractor/service details, contract number, date/time verification, and signature of Health Services Administrator or designee.

• The contractor must ensure employees are free of communicable diseases and may be required to submit to testing for tuberculosis or other communicable diseases.

• The contractor must adhere to all institution regulations for safety, custody, and conduct of inmates.

• The contract employee will be required to operate a system of records for medical treatment documentation and adhere to the Privacy Act of 1974.

• The schedule consists of 12 visits per year, one visit per month.

• Visits are generally scheduled between 7:00 am and 3:00 pm, Monday through Friday, excluding Federal Holidays.

• Place of Performance: FMC Laboratory, FMC Lexington, 3301 Leestown Rd., Lexington, Kentucky 40511.

• Qualifications for the Laboratory Director include MD, DO, or DPM licensed to practice, with specific board certifications or equivalent experience, or a Doctoral degree in a relevant science with board certification.

• The contractor must provide candidate qualifications, including licenses, diplomas, certifications, letters of recommendation, and resumes.

• The contractor must wear appropriate and professional attire, and is prohibited from wearing khaki or green clothing.

• The service is a contractual arrangement, not a personnel appointment, and does not constitute an employer/employee relationship.

• The contract employee may be required to undergo contractor orientation and security-related training.

• The contract employee may be removed from the institution at the discretion of the CEO.

• The contractor must provide a six-month notice if seeking dissolution of the contract.

• Provide Laboratory Director Services in accordance with the attached Statement of Work.

• The contract includes a base year and option periods, totaling up to 5.5 years.
