The acquisition is for Food and Medical Waste Collection and Disposal Services for the Defense Health Agency (DHA) Healthcare Contracting Division – West (HCD-W) at Commander, Fleet Activities Chinhae (Chinhae, Republic of Korea (ROK)) Medical Clinic BLDG. 502.

• Provision of facilities custodial services, food waste collection and disposal services, and medical waste collection, transportation, and disposal services.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for custodial, food waste collection and disposal, and medical waste collection, transportation, and disposal services at Commander, Fleet Activities Chinhae (Chinhae, Republic of Korea (ROK)). The government will not exercise supervision or control over the contractor's personnel.

• Custodial Services: Maintaining cleanliness and appearance of facilities, including daily disinfection of frequently touched surfaces, emptying waste and recycling containers, low and high area cleaning, interior and exterior window cleaning, blind cleaning, entrance cleaning, interior glass surface cleaning, drinking fountain cleaning, and lunch/break room cleaning.

• Food Waste Collection and Disposal: Collecting and disposing of food waste in accordance with federal, Korean, and local laws. This includes providing suitable containers, maintaining them, and developing a collection and disposal schedule.

• Medical Waste Collection, Transportation, and Disposal: Collecting, transporting, and disposing of medical waste in compliance with Korean Environmental Governing Standards (KEGS) and other applicable laws. This includes developing safety plans, manifesting waste, and ensuring proper disposal.

• Services must be performed at specified frequencies and meet Common Output Level of Service (COLS) standards.

• Performance is evaluated using a Quality Assurance Surveillance Plan (QASP).

• Acceptable Quality Levels (AQL) are generally set at 98% for most services, with 100% for medical waste.

• Remedies for failure include re-accomplishment within 4 hours.

• Services are to be performed at Commander, Fleet Activities Chinhae (Chinhae, Republic of Korea (ROK)).

• The contract period includes a base period plus four option years, with a potential six-month extension.

• Offerors must possess or obtain requisite ROK business licenses and comply with all applicable ROK laws.

• Pricing must be provided in Korean Won.

• Contractors must comply with safety, security, and information safeguarding requirements (e.g., OSHA, CUI, OPSEC).

• Personnel qualifications, including a Contract Manager with at least two years of experience, are specified.

• A Quality Control Plan (QCP) must be established and implemented.

• Specific training requirements, including Bloodborne Pathogens training, are mandated.

This document is a Past Performance Questionnaire (PPQ) provided by the Defense Health Agency (DHA) Western Markets Contracting Division. It is used to gather feedback on an Offeror's past performance for a U.S. Government contract. The completed questionnaire is intended to be submitted by the Offeror with their quote, though it can also be returned directly to the DHA.

• Evaluator Information: Fields for the name, agency, address, phone, email, and position of the evaluator.
• Rating Scale: A standardized rating system (Exceptional, Very Good, Satisfactory, Marginal, Unacceptable, Not Applicable) to evaluate various aspects of performance.
• Performance Areas: Specific questions covering client relationships, subcontractor management, hiring/retention, corporate management, quality of work, quality control, schedule adherence, cost control, compliance (labor, safety), and timely documentation submission.
• Contractual Issues: Questions regarding cure notices, show cause letters, and problem-solving capabilities.
• Re-award Decision: A question asking if another contract would be awarded to the evaluated firm.
• Customer Satisfaction: Inquiry about satisfaction with the end product.
• Opportunity to Respond: Confirmation if the firm was given a chance to respond to negative comments.
• Overall Rating: A final overall performance rating.

Bidders should ensure this questionnaire is provided to relevant past clients or partners who can provide a positive and accurate assessment. The feedback gathered through this PPQ will be a factor in the evaluation of proposals. Bidders should also be aware that the government reserves the right to verify all information provided.
