Statement of Work (SOW) / Performance Work Statement (PWS)

To establish a reliable and responsive maintenance program for six (6) Defense Logistics Agency (DLA) fork trucks at Rock Island, ensuring continuous availability for mission operations. This includes the immediate repair of one currently disabled TCM fork truck (Model FCG25N6, Serial Number 48802042).

• Provide all labor, tools, equipment, materials, parts, and services for preventive and corrective maintenance of six fork trucks.

• Services must comply with applicable OSHA, ANSI, and manufacturer standards.

• The contractor shall verify repairs for the disabled fork truck and provide a written price quotation prior to performance.

• Services will primarily be performed at the Contractor's facility, with Government delivery and retrieval of fork trucks, unless on-site service is authorized.

• Preventive Maintenance (PM) to be performed quarterly, consistent with manufacturer recommendations.

• Service Response: Acknowledge service requests within two (2) business hours.

• Unscheduled Repairs: Provide written estimate within one (1) business day of inspection/diagnosis.

• Emergency Repairs: Contact Government POC within one (1) business hour of receipt and prioritize diagnosis/scheduling.

• Documentation: Maintain a service log for each fork truck and provide a quarterly summary report.

• Period of Performance: Up to thirty-six (36) months, consisting of one (1) twelve (12)-month base period and up to two (2) twelve (12)-month option periods.

• Location: Rock Island Arsenal, DLA Document Services – West Region.

• Use new or remanufactured OEM parts or OEM-authorized equivalent parts.

• Technicians must have a minimum of five (5) years of experience servicing specified fork truck models or equivalent.

• Technicians must be eligible for access to Department of Defense (DoD) facilities.

• Invoices to be submitted electronically via Wide Area Workflow (WAWF) or successor system.

Amendment / Modification

• This amendment revises two attachments related to the solicitation.
