Statement of Work (SOW) / Performance Work Statement (PWS)

• Rental of four (4) forklifts for a base period of 12 months, with a 6-month option period.

• The contractor must provide all maintenance, repairs, and support to ensure continuous safe operation.

• Forklift specifications include: Class 5, Lift Code 4, internal combustion (LPG), counterbalanced, standard 4 ft tines, 3-ton or greater capacity, with a safe working load lift capacity of 6000 lb - 7000 lb. Tires should be air-filled or solid pneumatic.

• The contractor must ensure equipment is delivered in good working order, free of defects, and maintained safely. All safety features must be functional.

• Unlimited hourly usage is permitted.

• Propane tanks are to be provided as required.

• Current inspection and maintenance records must be supplied.

• Base Period: June 2026 to May 2027

• Option Period: June 2027 to December 2027

• Work will be performed at various locations within PHNSY & IMF.

• Equipment delivery is required prior to the start of the base period.

• Equipment removal must occur within two (2) working days following the conclusion of the final performance period.

• Coordination with the Contracting Officer's Representative (COR) and Technical Point of Contact (TPOC) is required for delivery and removal within the Controlled Industrial Area (CIA).

• Equipment Availability: 95% availability per month.

• Response to Equipment Failure: Provide a replacement unit within 24 hours of notification. No more than one late replacement per 6-month period.

• Scheduled Maintenance Compliance: 100% on-time completion of scheduled maintenance events.

• Applicable documents include OSHA standards (29 CFR 1910.178), manufacturer safety and maintenance requirements, and state/local transportation regulations.

• PHNSY&IMF will conduct pre-use inspections covering tires, rims, fluids, leaks, battery, fuel system, engine noises, lights, horn, hoist, tilt, transmission, directional controls, brake system, gauges/meters, fire extinguisher, operator restraint system, forks, fork positioning locks/stops, fork safety chains, overhead guards, ground straps/static conductive tires, structural integrity, and mandatory markings.

• Any operational issues discovered during pre-use inspection must be remedied or replaced within 24 hours.

• The contractor shall provide all manufacturer-recommended preventive and unscheduled maintenance.

• A Preventative Maintenance Schedule must be submitted for COR approval within ten (10) days of contract award.

• Maintenance requests must be responded to within two (2) hours.

• If repairs cannot be completed within eight (8) hours, a comparable replacement forklift must be provided within 24 hours.

• Contractor commercial terms and conditions do not apply unless expressly incorporated.

• Government contract terms take precedence in case of conflict.

• Government will ensure only licensed and trained personnel operate the forklift and will perform daily operator checks.

• Security background checks and access badges are required for contractor personnel.

• No Government-furnished utilities or services will be provided.
