This document provides an "On Site Visit Information Sheet" and an attached "Criminal History Check" form (NCIC form). The NCIC form is required for all offerors interested in attending an onsite visit. It must be submitted to the Contracting Officer (Jessica Hutchison at J2Hutchison@bop.gov) as soon as possible to allow time for the NCIC to be run and approved prior to the site visit date.

On Site Visit Information Sheet:

• Requirement: All offerors must complete an attached NCIC form.
• Submission: Turn in the NCIC form to the Contracting Officer.
• Approval: NCIC clearance is required before entry to the institution.
• Access Restriction: No offerors will be permitted inside the institution without an approved NCIC.
• Address: 201 FCI Lane, Glenville, WV 26351.
• Special Instructions: Enter the front lobby of the Administration Building, provide a valid driver's license/picture ID, and all items (including no cell phones or electronics) will be X-rayed and individuals will pass through a metal detector. Offerors will be escorted.

Criminal History Check (NCIC Form):

• Purpose: Authorizes the Federal Bureau of Prisons to obtain criminal history information.
• Required Information: Includes fields for Name, Address, Telephone Number, Aliases, Citizenship, Social Security Number, Date of Birth, Sex, Race, Height, Weight, Eye Color, Hair Color, and Place of Birth.
• Consequences of Non-Disclosure: Failure to provide information may result in denial of entry, denial of volunteer/contract status, or termination of employment.

This document is critical for any bidder intending to participate in an onsite visit. Completing and submitting the NCIC form accurately and by the required deadline is mandatory for gaining access to the facility for the site visit. Failure to comply will prevent participation in the site visit.

Solicitation / RFP / RFQ

This solicitation (RFQ) is for Elevator Repair, including contracted labor, equipment, supplies, and materials. Bidders must ensure they are quoting on the exact measurements and components specified.

• Elevator Repair services.

• Bidders must quote on specified measurements and components.

• Submission of Representations and Certifications is required, ensuring active and up-to-date registration in the System for Award Management (SAM.gov).

• Completion of Standard Form (SF) 1449, specifically blocks 17a, 20, 30a, 30b, and 30c.

• Inclusion of offeror name and UEI number in the header section of quote sheets.

• Inclusion of email address and UEI number in block 20 of SF 1449.

• Original signatures are required on submitted quotes.

• Award will be made on a line-item basis to responsive and responsible vendors offering the best value to the Government.

• Past Performance will be considered approximately equal in value to price.

• Quotations must be submitted via email to j2hutchison@bop.gov.

• Faxed bids or hand-carried quotes will NOT be accepted.

• Quotes must be submitted only to the email address provided.

• Quotes received after the deadline will be processed per FAR 52.212-1(f).

• Deadline: February 6, 2026, no later than 4:00 p.m. EST.

• Onsite Visit: January 28, 2026, at 1:00 pm EST. Inquiries for the site visit should be sent to TEJarvis@bop.gov or LHurley@bop.gov.

• Not specified in this document.

• Awardees not meeting RFQ requirements will be noted and utilized as Past Performance Information, potentially shared with other Bureau of Prison facilities.

Statement of Work (SOW) / Performance Work Statement (PWS)

To procure all necessary equipment, labor, materials, and incidentals for replacing and repairing elevator components at FCI Gilmer.

• Supply, replace, and install the following elevator components:

  • Non-periphery MCE control
  • Power unit motor
  • Key activated fixtures (Best Key flat type)
  • Car operating panel
  • GAL door operator
  • Hanger rolls
  • GAL Interlocks
  • GAL hoist door hangers
  • All new travel and hoist cable wiring
  • Car guide rollers (replaced or repaired)
  • Replace current leveling system with MCE leveling system
  • All electronics and electronic wiring

• Work hours are Monday - Friday, 7:00 AM – 3:00 PM, excluding Federal holidays. Any work outside these hours requires 5 days advance scheduling.

• The location is FCI Gilmer.

• Contractor Security/Working Regulations:

  • Contractors must be familiar with working hours and conditions in a correctional facility.
  • Time must be allocated for security considerations (check-in, inmate movements, lockdowns).
  • In case of emergency, contractors must secure work and equipment and will be escorted out.

• Delivery of Materials:

  • All materials/equipment must be processed and coordinated with the COR for temporary storage.
  • Deliveries accepted between 7:30 AM - 2:00 PM, Monday-Friday, excluding Federal holidays.
  • BOP staff will not receive or sign for deliveries; contractor presence is required.
  • Contractor is responsible for loading/unloading and must not impede institution entry/driveway.

• Storage of Materials and Equipment:

  • Materials/tools stored in a mutually agreed secure lockable gang box or similar.
  • Tools inventoried upon entry and exit.

• Safety and Personal Protective Equipment (PPE):

  • Contractors must provide and wear required PPE.
  • Ladders must be secured.
  • Extension cords must be in good condition and secured.
  • Lock-out/Tag-out procedures must be followed, requiring both Contractor and FCI Gilmer locks.

• Additional Considerations:

  • No cell phones, cameras, or two-way pagers allowed for contractors.
  • Jeans are permitted, but green or khaki clothing is prohibited.
  • Lunches consumed at the work site.
  • Smoking only in designated areas.
