To provide all necessary labor, equipment, transportation, materials, and support accessories to remove existing glue-down carpet, wall base, stair nosing, and transitions, and to furnish and install new carpet tiles, rubber base, and transitions in Building 565, 1st Floor.

• Removal of existing glue-down carpet, wall base, stair nosing, and transitions.

• Furnishing and installation of 24"x24" carpet tiles with "Tile Tabs" adhesive.

• Furnishing and installation of 4-inch rubber base and transitions to match existing.

• Stair nosing, riser, and treads to match existing in color and quality.

• Potential requirement to support an elevated work schedule for completion within 1 month of the start date.

• For the auditorium area, if approved, stair nosing, riser, and treads will match existing in color and quality.

• Demo existing flooring material, all transitions, and all wall base and stair nosing in designated areas.

• Dispose of all building material off MacDill AFB.

• Mechanical removal of flooring material and non-water base adhesives.

• Any additional preparation due to unforeseen conditions must be brought to the attention of the Contracting Officer Representative (COR).

• If Asbestos Containing Material (ACM) is encountered, work disturbing ACM must cease, and the COR must be immediately notified.

• Protect adjacent property, building, and contents from dust, dirt, or other materials.

• Daily cleanup of trash, debris, refuse, and garbage, placing it in appropriate containers.

• Building 565, 1st Floor.

• Work to commence within 30 days and be completed within 60 days of purchase order award.

• Contractor must immediately notify COR of delays due to mandated carpet goods suppliers.

To remove existing carpet tile, wall base, and transitions, and then furnish and install new Type VI carpet tiles and 4-inch rubber and vinyl/metal transitions in designated areas.

• Provide all necessary labor, equipment, transportation, and materials.

To remove existing VCT flooring, wall base, and transitions in Building 26, Avon Park AFR, and install new carpet tiles and 6-inch rubber base.

• Remove existing wall base and transitions.

• Lift or move furniture as needed for flooring work.

• Furnish and install 24"x24" carpet tiles with "Pressure-sensitive" adhesive.

• Furnish and install 6-inch rubber base and transitions to match existing.

• Demolish existing transitions and wall base.

• Inspect existing VCT for loose tiles; remove and fill gaps with approved patching compound if necessary.

• Prepare the VCT subfloor by stripping wax and floor finish, and ensuring it is free of dirt, oil, and grease. Leveling compound may be used if height differences or grout lines are significant.

• Protect adjacent property from dust and debris.

• Daily cleanup of trash, debris, and refuse.

• Work must be completed within 1 month from the start date.

• All building material from demolition must be disposed of off Avon Park AFR.

• If Asbestos Containing Material (ACM) is encountered, work must cease immediately, and the COR notified.

• Location: Building 26, Avon Park AFR.

• Start: Within 30 days of purchase order award.

• Completion: Within 60 days of purchase order award.

• "Pressure-sensitive" adhesive to be used.

• Notify COR immediately of any delays due to mandated carpet goods suppliers.

• The VCT floor must be free of wax before adhesive application.

• Any additional floor preparation due to unforeseen conditions must be brought to the COR's attention.

• Contract Identification: Details of the previous contract, including description of services, period of performance, dollar value, contractor's role, number of projects, types of flooring, and total square footage.
• Customer or Agency Identification: Name of the customer/agency and geographic scope of services.
• Respondent Identification: Name, title, contact information, and duration of work on the contract for the person completing the questionnaire.
• Performance Information: Ratings on various aspects of contract administration/management and project execution, using a scale from Exceptional to Unsatisfactory. Specific areas include responsiveness, ability to complete service at awarded price, compliance with facility rules, timeliness of project start/completion, quality of installation, and interactions with building occupants.
• Government Contracts Only: Information on contract terminations.

This questionnaire is sent to entities that have previously contracted with potential offerors. Bidders should be aware that their past performance on similar contracts will be evaluated. The feedback provided by respondents directly influences the source selection decision. The deadline for submitting responses is 10 calendar days after receipt of the letter.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (Request for Quote - RFQ) for flooring replacement and installation services for commercial products or services. The acquisition is set-aside for small business concerns. The government intends to award a single Indefinite Delivery, Indefinite Quantity (IDIQ) contract with a ceiling value of $9,000,000.

• The scope includes all services and materials necessary for flooring installation, which may include replacement of existing flooring or initial installation.

Performance Work Statement (PWS)

This PWS outlines the requirements for flooring installation and removal services at MacDill AFB and Avon Park Air Force Range (APAFR).

• Services include installation, removal, and refinishing of various flooring types (carpet, VCT, LVP, LVT, rubber, ceramic/porcelain tile, wood base).

• Materials to be provided by the contractor, including flooring, adhesives, cove base, fasteners, etc.

• Contractor is responsible for all equipment, tools, labor, transportation, and off-base storage.

• Work includes moving and relocating standard office furniture and systems furniture.

• Removal of existing flooring, installation of new flooring, sanding/finishing of wood base, and removal/installation of stair treads and risers.

• Thorough cleaning of installation areas, including debris removal and surface preparation.

• Disposal of all waste materials and debris at a state-permitted site.

• Work will occur in occupied buildings, requiring coordination with occupants.

• Performance Objectives (PO) include removal of existing flooring, installation of new flooring, cleanup/disposal, furnishing materials, vacuuming, environmental requirements, and quality control.

• Performance thresholds are defined with "Excellent" (0 defects) to "Unsatisfactory" (more than 5 defects) ratings for most objectives.

• Surveillance methods include customer complaint and 100% inspection/periodic validation.

Amendment / Modification

• Technical Approach: Elaborates on the specific areas the technical approach must address, including processes for flooring replacement, waste disposal, base pass requests, moisture content verification, understanding of the Carpet II Program, management approach, and organizational structure.
• Past Performance Information: Specifies the requirement for 3-5 references and the information to be provided for each. It also clarifies the process for submitting Past Performance Questionnaires.
• Price Matrix: Outlines the structure of the IDIQ Price Matrix, including pre-established prices and rates, and pricing for sample task orders.
