Submission Requirements

Quotes must be submitted electronically via email to the listed POCs. Offers must be held firm for 150 calendar days. Submissions require three parts:

  1. Part I - Summary and Miscellaneous Data: Including CAGE Code, UEI, representations, and certifications.
  2. Part II - Price Quote: Submitted via the revised Attachment 1 (Price Exhibit), with accurate mathematical calculations.
  3. Part III - Past Performance: Include quality and satisfaction ratings, and up to five relevant contracts. Past Performance Questionnaires (PPQs) must be submitted by references no later than June 22, 2026, at 4:00 PM CT, specifying the volume of work (number of exhaust hoods cleaned per year).

Evaluation Factors
