Statement of Work (SOW)

To procure comprehensive mortuary services for unclaimed or indigent deceased Veterans under the care of the Veterans Affairs Palo Alto Health Care System (VAMC).

• Provide facilities, professional services, labor, supplies, equipment, storage, and transportation for handling unclaimed or indigent Veteran remains.
• Services include preparation (embalming, dressing, cosmetic beautification), cremation (inurnment), and burial.
• Ensure compliance with VA requirements, federal, state, and local health laws.
• Transport remains from VAMC or authorized care facility to a VA National or VA grant-funded State Veterans Cemetery.
• Contractor facility must be within a 25-mile radius of VAMC Palo Alto, CA.

Base Year: December 1, 2026 - November 30, 2027 Four (4) Option Years: December 1, 2027 - November 30, 2031

• Remains must be handled without discrimination based on race, creed, sex, or national origin.

• Professional services must be of the highest quality.

• Hygienic measures must ensure complete disinfection and sanitation.

• Response for removal of unclaimed remains within one (1) business day.

• Burial process to be completed within 10 days of notification.

• Contractor must maintain a Quality Control Program (QCP) and semi-annual reports.

• Government will use a Quality Assurance Surveillance Plan (QASP) for monitoring.

• Contractor must possess current transit, burial, cremation, or removal permits as required.

• Proof of establishment and funeral home director licenses required.

• Employees must have appropriate and current licenses.

• Contractor responsible for Workers Compensation and Professional Liability Insurance.

• Contractor must sign and adhere to confidentiality statements regarding VA sensitive information (PHI, PII).

• Annual security/privacy awareness training and signing of VA’s Rules of Behavior are required for employees accessing VA information.

• Casket requirements: standard size (min. 82x28 inches), 20-gauge or thicker metal, no leaks, no cardboard/pressed paper.

• Urn requirements: durable, sealed, not leaking (plastic, metal, wood, ceramic/stone acceptable).

• VA will provide the US flag for draping over the casket.

• VA will furnish the burial plot, marker, and grave opening/closing services at the designated cemetery.

• Contractor must coordinate frequently with the National or VA grant-funded Veterans State Cemetery and the COR.

• Contractor must provide proof of Veteran eligibility documents (e.g., DD 214) to the COR.

Sources Sought Announcement

This is a Sources Sought announcement for market research regarding Mortuary Services for unclaimed or indigent veterans at the VA Palo Alto Healthcare System (VAPAHCS). The government is seeking information to make acquisition decisions and is not soliciting proposals or quotes at this time. No costs incurred in responding will be reimbursed.

• Service: Non-Personal Services Contract for funeral (mortuary) services.

• Scope: Provide facilities, professional services, management, labor, tools, supplies, equipment, storage, and transportation for handling unclaimed or indigent remains of deceased veteran inpatients at VAPAHCS.

• Compliance: Offerors must comply with EPA/OSHA regulations.
