Scope of Work

- Facilitating Veterans' engagement in programming and communication with Case Managers.
- Screening and referrals for benefits, independent living, and social skills enhancement.
- Permanent or transitional housing search support, utilizing recovery-oriented and harm reduction approaches.
- Ensuring basic needs are met, including three daily balanced meals, an evening snack, laundry facilities, secure storage for belongings and medication, and access to food/clothing pantries.
- Providing transportation assistance for appointments and public transit education.
Provider & Facility Requirements
Providers must demonstrate capacity for outreach, secure separate housing and bathroom accommodations for males and females, and 24/7 access to assistance for emergencies. Case managers should have experience in social services or healthcare, particularly with chronic medical, mental health, and substance abuse issues, and maintain BLS/CPR certification. Facilities must comply with local and state occupancy permits, safety, health, sanitation, and ADA requirements, including fire safety codes (NFPA 101).
