Statement of Work (SOW) / Performance Work Statement (PWS)

To procure contracted residential services (CRS) for homeless Veterans on Oahu, Hawaii, as part of the Health Care for Homeless Veterans (HCHV) program. The goal is to provide safe, supportive housing and facilitate the transition to permanent housing.

• Provide transitional housing, meals, and case management for 5 beds (potentially increasing) for homeless Veterans eligible for VA healthcare.

• Services include 24/7 on-site staff, provision of 3 nutritious meals and a snack daily, laundry facilities, and transportation to medical appointments and community resources.

• Case management services to assist Veterans in obtaining medical, financial, and social resources, developing individualized Housing Support Plans (HSPs), and securing permanent housing.

• Maintain a minimum of 80% occupancy and aim for 90-100% quarterly.

• Ensure Veterans are admitted on the same day of referral, with temporary placement if immediate admission is not possible.

• Maintain individual case records in compliance with HIPAA and other relevant regulations.

• Achieve VA Central Office metrics: 55% or more Veterans discharged to permanent housing and 20% or less negative exits.

• Discharge to permanent housing within 90 days or less, with extensions requiring VA HCHV Liaison approval.

• Maintain 80% or higher occupancy rate.

• Timely submission of required documentation, including incident reports, discharge summaries, and monthly progress reports.

• Period of Performance begins August 1st, 2026.

• Services to be provided on Oahu, Hawaii.

• Contractor must have a minimum of one year of establishment on Oahu.

• Housing should be clean, safe, secure, and ADA accessible if possible.

• Staff must be trained in CPR, AED, substance abuse, mental health, and other relevant areas.

• Strict adherence to IT contract security, information confidentiality, and records management regulations is required.

• Background security checks for all personnel are mandatory.
