Scope of Work

- Navigating the Medicaid application process to completion, including long-term care and community-based services.
- Providing personal assistance and resources to VHAMEHCS staff, patients, and caregivers.
- Proactively following up on pending applications with Medicaid offices and ensuring timely processing.
- Assisting with Medicaid appeals when necessary.
- Meeting with patients/caregivers at bedside or virtually to obtain information and signatures.
- Obtaining Releases of Information (ROI) and ensuring they are scanned into medical records.
- Making initial outreach for inpatient referrals within 48 hours and outpatient referrals within 7 days.
- Submitting applications within 72 hours of receiving a signed referral form.
