Sources Sought Synopsis

This document is a Sources Sought Synopsis for market research to identify potential vendors capable of providing Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) Licensed Treatment Facility services in Sacramento, Placer, or Yolo County for the VA Northern California Health Care System (VANCHCS). This is not a solicitation, and no contract will be awarded based on this notice. The government is not obligated to reimburse costs associated with responding.

• Provide emergency housing and supportive services for homeless Veterans.

• Services should align with HCHV CERS model requirements.

• Contractor must have a facility located in Sacramento, Placer, or Yolo County.

• Focus on providing a 30-90 day stay or longer.

• Offer case management support for income improvement, identification, housing applications, account access, and discharge planning.

• Provide 3 meals a day.

• Manage behavioral issues and substance use.

• Coordinate with the VA.

• Staff must be on-site 24 hours a day.

• Facility must be wheelchair accessible.

• Support homeless individuals with co-occurring disorders.

• Place of Performance: VA Northern California Healthcare System: Sacramento, Placer or Yolo County.

• Period of Performance: Intended for 5 years, potentially beginning around April 1, 2026.

• Pricing: Vendors are asked to provide estimated pricing for HCHV Services for market research purposes only.

• Vendors should indicate their socio-economic status (Small Business, 8(a), HUBZone, SDVOSB, VOSB, etc.).

• Limitations on Subcontracting: For small businesses, at least 50% of the cost of contract performance incurred for personnel must be expended for employees of the concern (FAR 52.219-14).

• A draft Performance Work Statement (PWS) and PWS Addendum 1 (Zip Codes) are attached for detailed requirements.

• The Service Contract Act will apply.

• Invoices will only be accepted on a monthly basis.

Performance Work Statement (PWS)

To provide emergency housing and supportive services for homeless Veterans in Sacramento, Placer, or Yolo County, California, through the Health Care for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) program. The goal is to remove homeless Veterans from unsafe conditions and place them in community-based residential environments with supportive services to improve their health status and housing situation, ultimately facilitating a transition to permanent housing.

• Provide safe, secure, and wheelchair-accessible housing for 6-12 homeless Veterans (initially 8 beds).

• Target and prioritize homeless Veterans eligible for VA healthcare, including those transitioning from chronic homelessness, institutional discharge, or recent homelessness.

• Facilitate same-day admission for referred Veterans.

• Accept referrals without requiring sobriety as a condition of admission, providing social model withdrawal management if needed.

• Offer supportive services including case management, care coordination, vocational services, benefits assistance, independent living skills enhancement, and permanent/transitional housing search support.

• Ensure Veterans engage with case managers and available programming.

• Emphasize placement in permanent housing and reduction of negative exits due to rule violations.

• Provide three nutritious meals daily and maintain laundry facilities.

• Maintain a minimum of 80% occupancy.

• Develop and update a collaborative care plan within 14 days of admission.

• Conduct case management meetings at least three times per month.

• Provide secure medication and belongings storage.

• Promote achievement of stable, independent housing (target 35%+ exits).

• Monitor and minimize negative exits (target 30% or less).

• Maintain 80% occupancy (preferred 90-100%).

• Achieve 35%+ (preferred 50%+) exits to permanent, independent housing.

• Limit negative exits to 30% or less (preferred 20% or less).

• 60% of contractor time dedicated to direct Veteran clinical services.

• 40% of contractor time dedicated to administrative services.

• Location: Sacramento, Placer, or Yolo County, California.

• Period of Performance: Begins April 1st, 2026.

• Length of Stay: Typically 30-90 days, with extensions based on clinical need.

• Contractor must be capable of providing 24/7 services.

• Staffing includes on-site administrative staff, case management/counseling personnel, and CPR-certified security/staff.

• Maintain individual case records in compliance with HIPAA, 42 CFR Part II, and 38 USC 7332.

• Report critical incidents immediately to the VA Liaison.

• Submit various deliverables including a Quality Control Business Plan, Staffing and Service Plan, Care Plans, Critical Incident Reports, Discharge Reports, Invoices, and Quarterly Performance Reports.

• Facilities must comply with ADA, fire safety codes (NFPA 101), health, and sanitation standards. Facilities will be inspected by a VA team prior to award and annually.
