Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the requirements for a contractor to provide residential programming and supportive services as part of the Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) Program. The goal is to offer community-based, short-term residential services to help homeless Veterans stabilize in the community and transition to permanent housing.

• Provide safe and secure housing for up to nine (9) Veterans, available 24/7.

• Offer supportive services including case management, care coordination, mental health stabilization, substance use disorder treatment, vocational assistance, and linkage to entitlements and budgeting.

• Assist Veterans in securing permanent housing through direct assistance, developing housing plans, and connecting them with resources.

• Provide access to essential amenities such as beds, private/shared bathing and toileting facilities, personal storage, laundry, and hygiene kits.

• Ensure access to prepared meals (at least three nutritious meals and snacks daily) with accommodations for dietary restrictions and preferences.

• Facilitate communication between Veterans and VA providers/emergency services.

• Support Veterans in obtaining tuberculosis (TB) clearance within 10 business days of admission.

• Maintain a minimum occupancy rate of 80% (90-100% preferred).

• Target values for exits to permanent housing (e.g., 55% or more).

• Target values for negative exits (e.g., 20% or less).

• Adherence to specific reporting requirements (e.g., daily census, critical incident reports, quarterly performance reports).

• Services to be provided in the City and County of San Francisco, CA, preferably in the Bayview-Hunters Point neighborhood.

• Contract covers up to five years, consisting of five 12-month Ordering Periods.

• Contractor must not deny entry based on factors like length of abstinence, number of treatment episodes, income, or pre-existing diagnoses.

• Admissions decisions should accommodate same-day admissions when vacancies exist.

• Contractor must maintain individual case records in compliance with relevant privacy laws (HIPAA, 42 CFR Part II, 38 U.S.C. § 7332).

• Strict adherence to computer security, data protection, and incident reporting protocols is required.

• Contractor staff must be trained in motivational interviewing and de-escalation techniques.

Sources Sought Notice

This notice is issued by the Department of Veterans Affairs, Network Contracting Office (NCO) 21, for market research purposes. The San Francisco Veterans Affairs Health Care System is seeking information to identify potential vendors capable of providing residential programming and services for the Health Care for Homeless Veterans (HCHV) Contract Emergency Residential Services (CERS) Program. This is not a solicitation or a request for quote/proposal.

• Provide community-based, short-term residential services with supportive services for stabilization and transition to permanent housing.

• Services to be offered in the City and County of San Francisco, preferably in the Bayview-Hunters Point neighborhood.

• Place of Performance: San Francisco, CA

• Period of Performance: Approximately 5 years, potentially beginning November 1, 2026.

• Vendors are also requested to complete a Draft Schedule with estimated pricing (for market research only), submit a capability statement, and include any relevant comments.

• The notice seeks to identify vendors regardless of business size.

• Information on socio-economic status is requested.

• Limitations on Subcontracting: For service acquisitions, small businesses (including SDVOSB and VOSB) must not pay more than 50% of the contract amount to subcontractors that are not similarly situated entities.

• Responses will be used for acquisition decision-making.

• The Government will not pay for any costs associated with responding.

• This notice does not commit the Government to a solicitation or contract award.
