Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) establishes the requirements for contract residential services (CRS) for the Health Care for Homeless Veterans (HCHV) program. The primary goal is to reduce homelessness among Veterans by providing short-term emergency residential placement, outreach, and engagement in treatment and rehabilitation services.

• Provide temporary, short-term housing (up to six months) for homeless Veterans.

• Offer outreach, case management, and rehabilitative services.

• Provide basic mental health services as needed.

• Support the VA's goal of preventing homelessness and engaging Veterans in treatment and other support services.

• Services are intended for homeless Veterans in specific geographic areas served by VA Eastern Colorado, VA Montana, and VA Cheyenne Health Care Systems.

• Facilities must pass initial and annual VA inspections per VHA Handbook 1162.04(1) Appendix B.

• Contractor must comply with all federal, state, and local laws.

• Staffing must include sufficient professional staff on duty or residing on-site 24/7.

• Contractor must adhere to patient rights provisions (38 CFR 17.33).

• Provide sanitary food preparation and at least three nutritious meals daily, plus snacks, accommodating dietary restrictions and cultural preferences.

• Provide secure medication storage.

• Maintain records according to the VA Records Control Schedule (RCS) 10-1.

• Submit quarterly reports on program activities, including veteran numbers, completion rates, and outcomes.

• Services are for Veterans located within the service areas of VA Eastern Colorado Health Care System, VA Montana Health Care System, and VA Cheyenne Health Care System.

• Housing duration for Veterans is typically 60 to 90 days, with a maximum of 180 days, unless prior approval is obtained.

• Contractors must operate residential facilities with demonstrated experience serving homeless individuals.

• Facilities should ideally be centrally located near public transportation and employment areas.

• Veterans must be screened for eligibility and suitability, with referrals made by VA HCHV Coordinators or designated employees.

• Contractors must manage incidents, including critical incidents, and notify the COR/HCHV CRS Liaison within 24 hours or the next business day.

• Secure and provide file plans for veterans' records, including location, admission, and discharge dates.

This notice is for market research purposes only to identify potential sources for future Health Care for Homeless Veterans (HCHV) Contracted Residential Services. It is not a solicitation, and no contract will be awarded from this notice. The Department of Veterans Affairs (VA), VISN 19, and NCO 19 are seeking information on qualified sources capable of providing these services in specific geographic locations.

• Provide Health Care for Homeless Veterans (HCHV) Contracted Residential services.

• Services to be provided in Metro Denver (CO), Fort Collins (CO), Bozeman (MT), Kalispell (MT), and Missoula (MT).

• Facilities should ideally be in centralized locations, near public transportation, and employment areas.

• Services include outreach, engagement in treatment and rehabilitation, coordination with VA clinical staff, and therapeutic rehabilitative services.

• The government contemplates awarding up to 5 contracts, one for each location.

• Period of Performance: Contemplated as one (1) base year with four (4) option years.

• Interested parties should submit a statement of capability outlining past relevant work.

• Responses are limited to five (5) single-sided, 12-point font, 8.5 x 11-inch pages.

• Capability statements must include: Vendor Name, Address, POC, Contact Info, CAGE Code, Business Category, Company Size, Brief Introduction, Teaming Arrangements, Relevant Contract Experience (Government/Commercial with details), Current Price List, and Ability to meet VA requirements.
