Submission Requirements
Interested parties must submit a response via email to Stephen.Silversmith@ihs.gov with the subject line “SSN-IHS1523171”. Submissions must include, at a minimum:
- Company Profile: UEI, Name and Address, Primary POC, business size, type of ownership, socio-economic status (e.g., 8(a), WOSB, SDVOSB, HUBZone), identification of Native American ownership, SAM.gov representations and certifications, and other related documentation.
- Capability Statement: A brief statement demonstrating the company's ability to meet the requirement, including current/past performance and established contracts (e.g., GSA, VA Schedule).
- IEE Representation Form: Indian-owned Small Business Economic Enterprises (ISBEEs) and Indian-owned Economic Enterprises (IEEs) must also submit a completed IHS IEE Representation Form (attached to the original notice) in accordance with HHSAR 326.606-1(b). This form is a self-certification of meeting the definition of an "Indian Economic Enterprise" under the Buy Indian Act.
