This is a combined synopsis/solicitation for commercial products and services to procure training and preventative maintenance services for Government-Owned Anesthesia Equipment for the New Mexico Veterans Affairs Healthcare System (NMVAHCS). The government is seeking quotes for services detailed in the Statement of Work.

• Preventative maintenance, inspection, and repair services for specific GE anesthesia equipment models (Avance CS2 Units, Carestation 650 A1 Units, Carescape Respironics Modules).

• Contractor-funded/provided technical training services for GE Avance CS2 and Carestation 600 Series.

• Emergency repair services (unpriced, requires contract modification).

• Use of Original Equipment Manufacturer (OEM) parts for all repairs and preventative maintenance.

• Next-day shipping for required parts.

• Response to emergencies within 4 hours.

• Provision of a replacement or loaner unit within 48 hours if the original cannot be repaired.

• Evaluation Factors: Award will be made to the responsible offeror whose quote, conforming to the solicitation, quotes the lowest price, provided the price does not exceed available funding and is found to be reasonable. Price alone is the sole evaluation factor.

• Special Standards of Responsibility: Offerors must provide current OEM certifications, three or more years of experience performing similar services, and access to OEM parts.

• This solicitation is set-aside for all Small Businesses.

• NAICS Code: 811210, with a small business size standard of $34M.

• This announcement constitutes the only solicitation; a written solicitation document will not be issued.

• Offerors must complete annual representations and certifications electronically via SAM.gov.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide preventive maintenance and repair services for Government-Owned Anesthesia Equipment used for patient care at the New Mexico Veterans Affairs Healthcare System (NMVAHCS).

• Perform complete inspections, vaporizer efficacy testing, and calibration for all anesthesia equipment listed in the contract, following manufacturer's recommendations and using provided Service Manuals.

• Conduct semi-annual preventive maintenance for Carestation 650s and Avance CS2s.

• Conduct annual preventive maintenance for Carescape Respironics Modules.

• Perform efficacy tests semi-annually for LCS vaporizers.

• Provide repair services for Carestation 650s, Avance CS2s, and Carescape Respironics Modules, including all parts, labor, and travel.

• Use only Original Equipment Manufacturer (OEM) parts for all repairs and maintenance.

• Provide next-day shipping for required parts to the facility.

• Respond within 4 hours to any emergency regarding the equipment.

• Provide a replacement or loaner unit within 48 hours if a unit cannot be repaired on-site.

• Verify that all serviced equipment is fully functional before returning it to the facility.

• Provide a Field Service Report for all service activities.

• Service technicians must be certified by the OEM.

• The contract allows for the addition and removal of equipment with Contracting Officer approval.

• Provide two OEM Technical Training Sessions for Biomedical Staff on the Carestation 650 and Avance CS2 models.

• Telephone response time for Field Service Engineers (FSE) or Technical Support is within four (4) hours during normal business hours (7:00 am to 4:00 pm Mountain Time, Monday through Friday).

• FSE on-site within 48 hours of emergency modification notification.

• Field Service Reports required within two weeks of service.

• Guarantee of workmanship and parts for a minimum of 90 days.

• Period of Performance: One base year plus four, one-year options.

• Hours of Coverage: Normal hours are Monday through Friday, 7:00 am to 4:00 pm local time. Services outside normal hours require Contracting Officer approval.

• 4-hour emergency response time.

• 48-hour loaner/replacement.

• OEM certifications for technicians.

• Security requirements for unclassified information must be followed.

• Contractor requires Contracting Officer approval for any contractual relationships involving disclosure of information.

This document is an addendum to FAR 52.212-1, "Instructions to Offerors – Commercial Products and Commercial Services." It provides specific instructions and requirements for submitting offers in response to solicitation 36C26226Q0309.

• Offer Submission: Offers must be signed, dated, and submitted to the specified office by the exact time and date indicated in the solicitation. Minimum required information includes solicitation number, offeror's name/address/size status, Unique Entity Identifier (UEI), EFT indicator, responses to provisions, and a statement of agreement with solicitation terms.
• Period for Acceptance: Offerors agree to hold prices firm for 90 calendar days from the offer receipt date, unless otherwise specified.
• Late Submissions: Late offers, modifications, or revisions will generally not be considered unless received before award and acceptance would not unduly delay the acquisition. Late modifications favorable to the Government may be accepted.
• Contract Award: The Government intends to award without discussions, so initial offers should contain the best terms. The Government reserves the right to conduct discussions, reject offers, accept other than the lowest offer, and waive informalities.
• Quote Format: Quotes must be in PDF, use 12-point font on 8"x11" paper, and be compatible with Microsoft Office Software. The package must be labeled with the contractor's name, business address, SAM UEI, solicitation number, and point of contact information.
• Offer Due Date: Responses are due no later than June 30, 2026, 5:00 PM PT, submitted electronically to daniel.kneizeh@va.gov. The subject line must include "RESPONSE TO RFQ 36C26226Q0309_0001". Only one quote per offeror is accepted; alternative solutions are not requested.

• Basis of Award: Procurement conducted under RFO 12 procedures. Award will be made to the responsible offeror whose quote is conforming, quotes the lowest price, does not exceed available funding, and whose quoted price is reasonable.
• Sole Evaluation Factor: Price.
• Special Standards of Responsibility:
  • SP1: Current OEM certifications to perform Preventative Maintenance, Inspection, and Repair Services, and the ability to use OEM parts (no substitutes).
  • SP2: Three or more years of experience performing Preventative Maintenance, Inspection, and Repairs of GE Anesthesia Equipment, evaluated for similarity in size, scope, and complexity.
• Past Performance: Evaluated to assess risks, based on contracting officer's knowledge, customer surveys, CPARS, or other reasonable bases. A neutral rating will be given if no past performance exists.
• Evaluation Process: Lowest-priced offer will be evaluated for reasonableness. If reasonable, the offeror will be identified as "successful." The Contracting Officer will then determine responsibility. This process continues until an award can be made or determined not possible.
• Compliance Check: A preliminary review of offers will be conducted. Incomplete submissions will not be forwarded for evaluation.
• Award Without Discussions: The Government intends to evaluate quotes and award without discussions, but reserves the right to conduct them.
• Options: Quotes will be evaluated by adding the total price for all options to the basic requirement. Unbalanced or excessive option prices may result in an unacceptable quote.
