Statement of Work (SOW) / Performance Work Statement (PWS)

To provide on-site medical services for students at the Great Onyx Job Corps Civilian Conservation Center (JCCC), ensuring comprehensive healthcare that supports their academic, vocational, and life-learning goals.

• On-Site Medical Services: Provide medical care to all students, adhering to Federal, OSHA, HIPAA, state, and local laws, as well as American Medical Association guidelines.

• Services Include: Examinations, assessments, treatment, prescription issuance, immunizations, health condition monitoring, hospitalization recommendations, follow-up care, and prevention planning.

• Healthcare Approach: Emphasize efficient, cost-effective, evidence-based contemporary healthcare, focusing on early identification and diagnosis of medical problems, basic health care, health promotion, prevention, and education.

• Coordination: Integrate medical services with other health and wellness components on Center.

• Student Population: Serve at-risk youth and young adults, typically aged 16-24, who are financially disadvantaged.

• Specific Tasks: Conduct initial physical examinations, develop treatment plans, maintain detailed medical records (Student Health Record - SHR), treat minor and acute conditions, manage chronic conditions, provide family planning education, prescribe medications (including Schedule II in collaboration with counselors), participate in relevant committees (DAC, applicant review), maintain medical equipment, and report health-related matters monthly.

• Referral System: Establish a system for students requiring specialized or emergent medical care.

• Training & Quality Improvement: Attend required training, participate in continuous quality improvement activities, and assist in developing the Health Education Program.

• Coordination: Collaborate with various Center departments (residential, recreation, education, vocation, etc.) for integrated services.

• Adherence to all applicable laws, regulations, and guidelines.

• Maintenance of patient privacy and confidentiality (HIPAA).

• Timely and accurate medical documentation.

• Monthly reporting on health-related matters.

• Cooperation with Job Corps assessment teams.

• Positive student satisfaction survey results.

• Location: Great Onyx Job Corps Center, 3115 Ollie Ridge Rd., Mammoth Cave, KY 42259.

• Schedule: Services required on-site, typically 1-2 days per week, Monday-Friday, 8 am – 5 pm, with days and hours mutually agreed upon with Center personnel. Schedule may vary based on Center On Board Strength (OBS) and PRH requirements, with modifications made via contract amendment.

• Personnel Qualifications: Physician and/or Nurse Practitioner/Physician Assistant with active, unrestricted licenses to practice independently in the state of service. Ability to work independently and as part of a team.

• Supervision: Provide supervision to Nurse Practitioner and/or Physician Assistant as required by state regulations.

• Background Checks: Personnel must agree to criminal background checks and drug testing if deemed necessary by the Contracting Officer (CO).

• Insurance: Maintain malpractice/professional liability and general liability insurance.

• Government Furnished: The government will provide adequate workspace, examination rooms, equipment, supplies, and materials.

• Security: Contractor personnel must comply with Center security procedures.

• Data Rights: Student records and generated reports are the property of the US Government.

• Prohibited Activities: No research projects or publications related to Job Corps health activities without prior written approval.

• Reporting: Report adverse student behavior incidents promptly.

• Holidays: Contractor personnel will not be scheduled to work on specified Government holidays.
