Wage Determination

• Uniforms: The cost of required uniforms and their maintenance cannot be borne by the employee if it reduces their hourly rate below the determined wage. Reimbursement for cleaning and maintenance is required at a specified rate unless "wash and wear" materials are used and require no special treatment.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide on-site preventative maintenance and equipment repair services for gym equipment at the Gunfighter Fitness Center, Mountain Home AFB, Idaho.

• Provide all personnel, labor, supervision, management, supplies, tools, equipment, parts, materials, transportation, shipping, and delivery necessary for gym equipment maintenance.

• This contract covers equipment maintenance only and does not include civilian gym operations or facility personnel.

• Perform monthly and quarterly preventative maintenance (PM) as scheduled in Appendix A.

• Conduct on-site electronic tests and inspections, including lubrication, calibration, cleaning, and pad repair.

• Respond to unscheduled service calls within 24 hours of notification, with repairs completed within two (2) business days.

• Supply required replacement parts (new or refurbished) unless under warranty. Contractor is responsible for parts up to $500 per job. Government approval is required for parts exceeding this amount.

• Provide upholstery repair/replacement as needed, coordinating with the Government POC for equipment older than 5 years.

• Maintain a detailed maintenance repair log (physical or electronic) including service dates, work performed, costs, and parts installed.

• Prepare a service report for each service call, verified by Fitness Center staff.

• Manage equipment removal or addition as directed by the Contracting Officer (CO).

• All maintenance to be done according to manufacturer's specifications.

• Ensure all known/identified discrepancies are corrected or repaired during quarterly PM visits, with exceptions only for parts availability.

• Unscheduled service calls to be responded to within 24 hours, with repairs completed within two (2) business days.

• Quality Control Plan (QCP) to be developed, implemented, and maintained to ensure quality services.

• Normal hours of operation: Monday through Friday, 7:00 am to 4:30 pm MST.

• Services are to be performed at the Gunfighter Fitness Center, Mountain Home AFB, Idaho.

• Contractor personnel must be able to read, write, speak, and understand English fluently.

• Personnel must present a neat appearance and be easily identified as contract employees.

• Base identification and vehicle passes are required.

• Contractor personnel must comply with all security, OPSEC, and antiterrorism requirements, including background checks and training.

• Hazardous material handling and disposal must comply with federal, state, and local regulations.

• Contractor is responsible for hazardous waste determination and disposal.

• Historic preservation coordination is required.

• Contractor employees are prohibited from possessing weapons, firearms, or ammunition on base.

• Specific equipment maintenance exclusions: floating equipment, barbells, weight plates, and smaller supply items like cable attachments and resistance bands are not the contractor's responsibility.

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for Gym Equipment Maintenance services. It serves as the only solicitation, and proposals are being requested in lieu of a separate written solicitation. The acquisition is being issued as a Request for Quotation (RFQ).

The solicitation requires maintenance for fitness center equipment, including:

• Monthly Preventative Maintenance

• Quarterly Preventative Maintenance

• Replacement Parts (estimated)

• Unscheduled Repairs/Emergency Services

• Contract Type: Firm Fixed-Price

• Period of Performance: Base Year (17 June 2026 - 16 June 2027) with four (4) Option Years extending through 16 June 2031.

• Technical Capability will be evaluated on a pass/fail basis using descriptive literature.

• Past Performance will be evaluated using a rating scheme based on CPARS, FAPIIS, and SPRS. Only "Acceptable" ratings will be further evaluated.

• Evaluation Factors: The Government will award to the responsible offeror whose offer is most advantageous, considering Price, Technical Capability, and Past Performance.
