Statement of Work (SOW) / Performance Work Statement (PWS)

This Statement of Work outlines the requirements for quarterly and annual ventilation surveys at the Erie VA Medical Center to ensure compliance with VA, Joint Commission, OSHA, and infection control standards, and to maintain environmental safety.

• Quarterly Ventilation Surveys: Conducted four times per contract year (September, December, March, June) in specified areas including Sterile Processing Service (SPS), Surgical Suites, Laboratory, Pharmacy, CLC Bravo, Urgent Care Center, Dental Clinic, Surgical Clinic, and Miscellaneous locations.

• Annual Ventilation Surveys: Performed annually for specific tests including Waste Anesthetic Gas Monitoring, N2O Leak Detection, Vacuum Terminal Performance, Nitrous Bank Leak Detection, and Medical Piping Systems Evaluation (CO, CO2, Halogenated Hydrocarbons, Methane, Dew Point).

• Surveys evaluate air flows, room pressure relationships, air exchange rates, and performance of critical environments.

• Reports must include room name, actual air exchanges, minimum air exchanges, room pressure, and a pass/fail status.

• A preliminary report is required during the survey.

• A final report is required after each quarterly and annual survey, including date, results, recommendations, signature, and test data.

• Place of Performance: Erie VA Medical Center, 135 East 38th Street, Erie, PA 16504.

• Schedule: Services performed Monday through Friday, 7:00 am to 4:30 pm, unless otherwise approved. Contractor must confirm inspection dates with the Engineering Program Manager at least 48 hours in advance.

• On-Site Requirements: Contractor representatives must sign in with Police, obtain an ID badge, and notify the Engineering Program Manager upon arrival and departure.

• Training: All contractor employees requiring access to VA information must complete VA Privacy and Information Security Awareness training and annually complete required security training. Training certificates must be submitted prior to work. Records management training is also required.

• Records Management: Contractors must comply with applicable records management laws and NARA policies, including preservation and safeguarding of Federal records. Unauthorized destruction or removal of records is prohibited. Any disclosures of information must be reported immediately. Government owns all data and records produced.
