Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for hazardous waste testing, packaging, and transportation services for various U.S. Army installations on the Island of Oahu, Hawaii. The goal is to ensure proper management and disposal of hazardous waste in compliance with all applicable regulations.

• Services: The contractor shall provide all necessary management, transportation, labor, equipment, materials, supplies, and tools for testing, sampling, packaging/repackaging, and transportation of hazardous waste.

• Compliance: All activities must adhere to Occupational Safety and Health Administration (OSHA) standards, Federal, State, and County laws, Environmental Protection Agency (EPA) regulations (including 40 CFR 261, RCRA), Department of Transportation (DOT) regulations, and other relevant authorities.

• Waste Types: Includes waste solvents, fuel, paint, oil, and acids.

• Testing: Sampling and testing to determine hazardous characteristics (ignitability, corrosivity, reactivity, toxicity) according to EPA standards.

• Packaging: Packaging or repackaging waste into DOT-specification containers, including pumping from storage tanks or separators.

• Transportation: Transporting hazardous waste to Department of War (DOW) accredited testing facilities.

• Documentation: Preparing and submitting required documentation, including Hazardous Waste Profile Sheets (TE 2), test reports, and chain of custody forms.

• Work Request Acknowledgment: Within 3 business days (90% AQL).

• Sampling Completion: Within 7 working days of Work Request (95% AQL).

• Container Processing: Within 3 working days of Work Request (100% AQL).

• Analysis Completion: Within 14 working days of sampling (100% AQL).

• Report Submission: E-copy within 21 working days of Work Request (100% AQL).

• Transportation Completion: Within 14 working days of Work Request (100% AQL).

• Compliance: All tasks performed in accordance with EPA, HDOH, DOT, and applicable laws (100% AQL).

• Location: Various Army installations on the Island of Oahu, Hawaii.

• Work Hours: Monday through Friday, 9:00 AM to 3:00 PM, excluding Federal Holidays.

• Personnel: All personnel, including the Project Manager, must hold current HAZWOPER and RCRA training certifications. Key personnel require a baccalaureate degree or equivalent experience.

• Security: Contractor personnel must follow installation access procedures, possess valid identification, and are subject to background checks.

• Environmental Compliance: Strict adherence to environmental regulations, including waste management, pollution prevention, and spill response. Contractor is liable for environmental fines resulting from their operations.

• Quality Control: Contractor must develop and maintain a Quality Control Plan (QCP).

• Deliverables: E-copy of Hazardous Waste Profile Test Report (A001), Hazardous Waste Profile Sheet (TE 2) (A002), and Quality Control Plan (A003).

This document, DLA Form 2511 (formerly DRMS Form 1930), is a Hazardous Waste Profile Sheet. It is used to characterize and document hazardous waste streams for disposition. Bidders may need to complete or review this form to understand the nature of waste materials associated with an opportunity.

• Part I: General Information: Generator details, facility address, EPA ID, waste profile number, and process generating the waste.
• Part I: Waste Information: Waste name, US EPA waste codes, state/local codes, projected annual volume, mode of collection, and information on dioxin listing, land disposal restrictions, and treatment standards.
• Part II: Material Characterization: Details on color, density, physical state, solids content, layering, and BTU/LB (if applicable).
• Part II: RCRA Characteristics: Identification of ignitable, corrosive, reactive, and toxicity characteristics, including specific test results or references.
• Part II: Chemical/Material Composition: A detailed list of components and contaminants, including CAS numbers, descriptions, concentrations, and ranges.
• Part II: Shipping Information: DOT hazardous material classification, proper shipping name, hazard class, and packing group.
• Part II: Special Handling Information: Description of any known or suspected hazards associated with exposure.
• Part II: Generator Certification: Certification by the generator that the information is accurate and all known hazards have been disclosed, supported by chemical analysis or user knowledge.
