Statement of Work (SOW) / Performance Work Statement (PWS)

This Scope of Work (SOW) details the requirements for the delivery and transportation of propane to the Hopi Health Care Center (HHCC) in Polacca, Arizona, and the IHS Employee Housing Compound Units in Keams Canyon, Arizona.

• The contractor shall provide all labor, materials, tools, equipment, and transportation for propane services.

• Propane is required for a 30,000-gallon tank (heating and hot water for HHCC) and a 12,000-gallon tank (for the HHCC emergency generator).

• Propane will also be supplied on an as-needed basis to empty/unoccupied housing units in Keams Canyon, AZ, and Polacca, AZ.

• Deliveries must be coordinated with the Technical Point of Contact (TPOC).

• The contractor is responsible for providing a receipt of delivery, including quantity.

• The contract period is from June 14, 2026, through June 13, 2031.

• This includes one Base Year (June 14, 2026 - June 13, 2027) and four Option Years.

• Delivery hours are Monday through Friday, 7:00 AM to 5:00 PM MST, excluding Federal holidays, unless otherwise agreed upon.

• The contractor must be certified and authorized to transport and deliver propane.

• Evidence of certification/licensure, including a Motor Carrier License from the Arizona Department of Transportation and applicable propane transport permits/certificates, must be provided and maintained throughout the performance period.

• Evidence of renewals must be provided upon demand.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ-IHS1522290) for commercial items, specifically the delivery and transportation of propane. The Indian Health Service (IHS) - Phoenix Area Office (PAO) requires these services for the Hopi Health Care Center in Polacca, Arizona, and IHS Employee Housing Compound Units in Keams Canyon, Arizona.

• Delivery and transportation of propane.

• The requirement is under NAICS code 424720 (Propane merchant wholesalers).

• The requirement is set-aside 100% Buy Indian, specifically for American Indian and Alaska Native Small businesses meeting the criteria of 13 CFR Part 121.

• Offerors must be registered in the System for Award Management (SAM).

• The propane contractor must be certified and authorized to transport and deliver propane, providing evidence of Motor Carrier License and applicable permits/certificates.

• All IT systems (hardware and software) must operate in a pure IPv6 network environment, complying with NIST USGv6 Program standards.

• The contract will be a purchase order.

• The period of performance includes a Base Year (June 14, 2026 - June 13, 2027) and four Option Years.

• Pricing will be evaluated in whole, including any optional accessories quoted.

• The contract may include options to extend services or the term, not to exceed 6 months for services and a total of 5 years for the contract term including options.

• Submit written quotes only; oral offers will not be accepted.

• The quote due date is March 2, 2026, at 1:00 PM MST.

• This acquisition will be awarded to the lowest priced responsive and responsible offeror on an "All or None" basis.

• Offerors must include a completed copy of FAR 52.212-3, Offeror Representations and Certifications - Commercial Items.

• 100% Buy Indian set-aside for American Indian and Alaska Native Small businesses.

• Offerors must be registered in SAM.

• Offerors must meet the definition of an Indian Economic Enterprise (IEE) and an Indian Small Business Economic Enterprise (ISBEE).

• This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2025-03 (dated 01/17/2025) and FAC 2025-06 (dated 10/01/2025).

• Invoicing will be processed through the Invoice Processing Platform (IPP).
