Performance Work Statement (PWS)

To provide dedicated, on-site Advanced Life Support (ALS) Ambulance services with crew for the 2nd HRF EXEVAL Exercise.

• Provide all personnel, equipment, tools, materials, supervision, and quality control necessary for dedicated ALS Ambulance services.

• The contractor shall provide three dedicated ALS Ambulances with Crew (6 EMT certified Personnel): two primaries and one in reserve.

• Services are required for the Urban Search and Emergency Response Facility Training Area at 1 Garrison Road, Annville, PA 17038, Fort Indiantown Gap.

• The reserve ALS ambulance and crew must be dispatched prior to the 1st crew leaving if a primary crew is unavailable.

• Period of Performance: 29 May 2026 – 31 May 2026.

• Hours of Service: 0700-1900 on Friday through Sunday.

• Location: Urban Search and Emergency Response Facility Training Area at 1 Garrison Road, Annville, PA 17038, FTIG.

• All crew members/contractors must have a valid REAL ID to enter and perform services in federal facilities.

• Contractor must comply with all applicable installation/facility access and local security policies and procedures.

• Contractor must provide information for background checks.

• Contractor and employees are subject to non-disclosure agreements regarding operational details.

• Photographing, video recording, and use of Commercial off-the-shelf Unmanned Aerial Systems (COTS-UAS) are prohibited without prior coordination and exception.

• All employees must receive local iWATCH training within 30 calendar days of contract award and within 30 days of new employees commencing performance.

• Contractor must protect Personally Identifiable Information (PII) in accordance with DFARS and DoD policies.

• Contractor shall safeguard all Government property provided for contractor use and secure facilities at the close of each work period.

• A Contract Manager (CM) and an alternate must be designated in writing to the Contracting Officer (KO).

• Contractor will have a working ambulance equipped for basic life support.

• Submit full names and DOB of all contractors entering site NLT 1 week prior to contract execution.

• The contractor shall ensure their equipment is serviceable at all times.

• The contractor shall be on standby to transport injured personnel to a local hospital.

• Government POC will check daily to ensure ambulance crew is on time and properly equipped.

• Government will evaluate performance in accordance with the Quality Assurance Surveillance Plan (QASP).

• The contractor must implement a Quality Control program.
