This document is a Past Performance Questionnaire (PPQ) intended to gather feedback from entities who have experience with a contractor's performance. It is to be completed by respondents and returned directly to the Contracting Office for Solicitation No. FA561326Q0006, which pertains to Human Performance Optimization (HPO) services.

• Contractor Information: Fields for the contractor's name, address, contact details, and a brief description of the work performed, personnel type, contract number, amount, period of performance, and place of performance.
• Respondent Information: Fields for the respondent's name, title, address, and contact details.
• Performance Ratings: A matrix with definitions for rating performance (Exceptional, Good, Satisfactory, Marginal, Unsatisfactory, Not Observed/Applicable) for specific performance areas.
• Performance Areas Assessed: Questions regarding service turnaround times, ability to comply with SOW/PWS, provision and retention of qualified personnel, communication and problem resolution, overall performance rating, issuance of cure/show cause notices, CPARS rating, and willingness to award another contract.
• Narrative Feedback: Sections for Strengths, Weaknesses, and Other Remarks.

Bidders should ensure their past performance is documented and that their references are aware of this questionnaire. The feedback provided by respondents will be used by the government to assess the bidder's past performance, which is a critical factor in the evaluation of proposals for the HPO services opportunity. The completed questionnaire is due by the offer due date of July 13, 2026, and should be returned to TSgt Carleen Clinger at carleen.clinger@us.af.mil.

This document is Attachment 2, a Pricing Worksheet for the RFQ FA561326Q0006 - 435th AGOW Human Performance Optimization (HPO) Services. Bidders must use this worksheet to provide their pricing details.

• Contractor Information: Requires contractor name, address (with ZIP+4), email, quote number, quote expiration date, telephone number, CAGE code, TIN, and SAM UEI.
• Payment Terms: Specify prompt payment terms or discounts.
• FOB Point: Must be "Destination".
• SAM Registration: Ensure SAM information is current and registration is not expired.
• CLIN Structure: All dollar values must be rounded to the nearest dollar or Euro. Extended prices must be exactly divisible by the quantity. Bidders should enable "set precision as displayed" in Excel to ensure accurate calculations.
• Points of Contact: Provide primary and alternate points of contact (Name, Title, Address, Telephone, Email).
• Statement of Understanding: Contractors must confirm they understand the requirements and will meet performance standards.
• Exceptions: Contractors must state whether they take exceptions to any requirements. If exceptions are taken, they must be listed with rationale.
• CLINs: The worksheet details CLINs for Certified Strength & Conditioning Specialist, Licensed Clinical Social Worker, Physical Therapist - Specialized, Operational Psychologist - Specialized, Training & Travel, and a 30-Day Phase-in Period. All CLINs are listed with a quantity of 12 months (or 1 lot for Training/Travel and Phase-in) and a Unit Cost and Total of 0.0, indicating these are fields to be filled by the bidder.

This worksheet is critical for bidders to accurately submit their pricing. Bidders must complete all yellow-highlighted cells and ensure their SAM registration is up-to-date. Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for Human Performance Optimization (HPO) services to support the 435th Air Ground Operations Wing (AGOW). The primary goal is to enhance physical performance, reduce injury risk, improve behavioral resilience, optimize cognitive performance, and increase the long-term durability of assigned personnel.

The contractor will provide integrated support in the following areas:

• Strength and Conditioning
• Physical Therapy
• Behavioral Performance, Resilience, and Operational Psychology

Services will be delivered in a manner that separates clinical and non-clinical activities. Medical services will be provided through individual clinical encounters documented in approved Medical Treatment Facility (MTF) systems. Non-clinical services will focus on performance optimization, coaching, and consultation, and will not involve clinical documentation or Protected Health Information (PHI) unless authorized.

The contractor must provide qualified personnel in the following labor categories:

• Three (3) Certified Strength and Conditioning Specialists (CSCS)
• Two (2) Physical Therapists (PT)
• One (1) Resilience and Performance Coach (RPC) with a Licensed Clinical Social Worker (LCSW) credential
• One (1) Operational Psychologist

The contractor is responsible for all personnel management, including recruitment, staffing, scheduling, supervision, and performance management.

• Human Performance Assessment: Conduct assessments within each provider's scope of practice. Clinical assessments by PTs and Operational Psychologists must be documented in MTF systems per HIPAA. Non-clinical assessments by CSCS, RPC, and Operational Psychologist are for performance optimization only and must not include PHI or Personally Identifiable Information (PII).
• Strength and Conditioning Services (CSCS): Focus on improving physical readiness, durability, injury prevention, and mission performance. CSCS will integrate with units, assess operational demands, and develop training strategies. They will not diagnose or treat medical conditions but will refer personnel to PT for injury concerns.
• Behavioral Performance Support (RPC): Provide non-clinical support for resilience, stress management, and performance optimization. This role is non-medical and does not involve diagnosis or psychotherapy. RPCs will conduct workshops and provide aggregate trend observations.
• Physical Therapy Services (PT): Provide one-on-one clinical care, including evaluation, treatment planning, rehabilitation, and therapeutic exercise. Services are based on provider availability and MTF scheduling. PTs will document all clinical encounters in approved MTF systems per HIPAA.
• Operational Psychology Services: Enhance human performance, resilience, and effectiveness through behavioral science. Services range from non-clinical performance enhancement to limited clinical intervention, with approximately 90% operational support and 10% clinical intervention. This includes leadership consultation, team building, assessment, and support for critical incidents.

Services are to be performed Monday through Friday, 0730-1630, excluding federal holidays, with potential adjustments for mission requirements. Services may be delivered at multiple locations throughout the Wing's area of responsibility.

The Quality Assurance Surveillance Plan (QASP) outlines the Government's approach to monitoring and evaluating contractor performance against established standards and performance objectives. Surveillance methods include routine monitoring, random sampling, customer feedback, trend analysis, direct observation, and record review.

Solicitation / RFP / RFQ

This solicitation (FA561326Q0006) is for Human Performance Optimization (HPO) Services for the 435th Air Ground Operations Wing (AGOW) at Ramstein Air Base, Germany. The contract will be a Firm-Fixed Price (FFP) Indefinite Delivery Indefinite Quantity (IDIQ) type, with services to be performed from September 17, 2026, through September 16, 2031. This is a commercial service purchase using RFO part 12 procedures and is not set aside for small businesses. The government desires delivery FOB Destination to Ramstein AB, Germany, by September 17, 2026.

The contract will cover the following CLINs:

• Certified Strength & Conditioning Coach (CSCS)
• Licensed Clinical Social Worker (LCSW)
• Physical Therapist - Specialized
• Operational Psychologist
• Training & Travel (Not to Exceed)

Detailed requirements are outlined in the attached Performance Work Statement (PWS).

• Page limitations apply to Volume I (10 pages) and Volume II (4 PPQS).

• Pricing information must be submitted in MS Excel format only.

• Evaluation will be based on Technical Capability, Past Performance, and Price, with Technical Capability being the most important factor.

• The government seeks the best value, which may not necessarily be the lowest priced offer.

• This acquisition is not set aside for small business concerns.

• A price evaluation preference will be applied for HUBZone small business concerns, unless waived.

• Offerors are required to meet all solicitation requirements. Failure to meet a requirement may result in an offer being ineligible for award.

• Multiple pricing offers will not be accepted.

• The government may order a maximum of 35 positions under this contract.
