This document is the "Past Performance Information Form" which contractors must complete to demonstrate relevant past performance or state that they possess no relevant past performance. It is used to gather information about previous contracts that are of similar scope, magnitude, and complexity to the current solicitation.

• Past Performance Requirement: Quoters must submit a maximum of three (3) current/previous contracts performed within the last three (3) years that are of a similar nature to the solicitation.
• Information Required per Reference:
  • Contract number, order number, or identifier
  • Contract type (Firm Fixed Price, Cost Reimbursement, or Time and Material)
  • Government agency or commercial entity name and address
  • Description of work/services provided
  • Point of contact (name, title, phone number, e-mail address)
  • Date of contract award
  • Period of performance
  • Initial and current contract value
  • Extent of subcontracting
  • Total dollar value broken into annual increments
  • Type of services provided and location(s)
  • Detailed description of work performed, demonstrating relevance to the solicitation's scope.
  • Information on subcontractors utilized, including the extent of work and annual dollar value.
• Experience: Quoters must describe recent experience (within the last 3 years) in performing the same or similar services of the same scope, magnitude, and complexity.
• Problem/Corrective Action: Quoters are encouraged to describe any problems encountered and corrective actions taken.

This form is critical for bidders to provide evidence of their capability and experience to perform the work described in the solicitation. It requires detailed information about past contracts, including specific tasks, performance periods, contract values, and points of contact. The government may contact these points of contact to verify past performance. Bidders should carefully select and describe their most relevant past contracts to demonstrate their suitability for the opportunity. For newly formed entities, information on key personnel and applicable resources should be provided.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial items, issued as a Request for Quotation (RFQ) for HVAC quarterly and monthly preventative maintenance services for NOAA Flower Garden Banks National Marine Sanctuary in Galveston, TX. The solicitation is a total small business set-aside.

• Non-personal services to provide labor, equipment, and materials for quarterly and monthly preventative maintenance on new HVAC mechanical equipment.

• Services include maintenance for Mitsubishi Cassette units (16), exhaust fans (2), outdoor units (7), and AHUs (7).

• Specific tasks include filter replacement, cleaning, inspection of various HVAC components, and overall system checks.

• A Statement of Work (SOW) is attached and incorporated by reference for detailed requirements.

• Contract Type: Firm-fixed-price.

• Period of Performance:

  • Base Year: August 1, 2026 – July 31, 2027 (12 months)
  • Option Year 1: August 1, 2027 – July 31, 2028 (12 months)
  • Option Year 2: August 1, 2028 – July 31, 2029 (12 months)

• Place of Performance: Flower Garden Banks NMS Building 216, 4700 Avenue U, Galveston, TX 77551.

• Quotes must be submitted via email to michelle.walton@noaa.gov by 5:00 P.M. MDT/MST on June 30, 2026.

• Offerors must provide separate line item pricing for the base year and all option years.

• Evaluation will be based on price and other factors, with a focus on technical approach/capability and past performance.

• Offerors must be registered in the System for Award Management (SAM) and provide their UEI#/CAGE CODE.

• A site visit is strongly encouraged. Dates are June 22-26, 2026. Contact Desda Sisson for appointments.
