Statement of Work (SOW) / Performance Work Statement (PWS)

The objective of this project is to construct an existing design of a 19-unit apartment building at a new location, ensuring compliance with all applicable local building codes and regulations. Site adaptation will include adjustments to architectural, structural, mechanical, electrical, and plumbing systems, along with site-specific considerations for landscaping and parking.

• The General Contractor (GC) will furnish all personnel, material, services, and facilities necessary for the construction as described in the construction drawings and specifications.

• Work will be performed at 516 E Indian Rte 6290, Tonalea, AZ 86044.

• The period of performance is 365 days.

• Work hours are Monday through Friday, 8:00 AM to 4:30 PM, with potential for extended hours if approved by the Contracting Officer.

• The contractor must coordinate operations to minimize disruptions to patient care services and the owner's day-to-day operations.

• Access to existing walkways, corridors, and adjacent occupied or used rooms must be maintained.

• Notification to the COR is required at least 72 hours in advance for activities affecting owner's operations.

• Utility interruptions require at least two days' advance notification and written permission from the COR.

• High noise, vibration, or odor-producing operations also require at least two days' advance notification and written permission from the COR.

• Smoking and controlled substance use are prohibited on the project site.

• Construction debris and waste material must be disposed of off-site at an EPA-approved facility; dumpsters at the facility are not available for contractor use.

• Submittals: Contractor must provide a complete schedule prior to Notice to Proceed (NTP). • Coordination with Occupants: The owner will occupy the site and adjacent rooms during construction. Coordination must go through the COR.

• Deliverables: Upon project completion, the contractor shall provide as-built drawings, O&M manuals, operational tests/reports/training/instructions, equipment lists, and final cleanup. Training for government staff on installed systems is required. Two copies of O&M manuals, parts lists, dealer contact information, operational tests, and warranty information are required. As-built drawings must be provided in hard copy and electronic format (AutoCAD and PDF for other documents).
