Solicitation / RFP / RFQ

This solicitation is for a Nationwide Construction Multiple Award Task Order Contract (MATOC) for Indefinite Delivery/Indefinite Quantity (IDIQ) services. The contract will provide a wide range of design-build (DB) and design-bid-build (DBB) services for new construction, renovation, alteration, and repair of medical and related facilities for the Department of Health and Human Services (HHS) / Indian Health Service (IHS). The work will primarily be performed in nine IHS geographic regions across the United States, with potential for work in Alaska, Tucson, and Bemidji areas. The competition is set aside 100% for Small Business. A "Seed Project" is attached and will be the first task order issued under this MATOC, with its scope of work and price proposal requirements detailed in the attachments.

• Services include general construction, renovation, alteration, and maintenance of medical facilities, including but not limited to major medical centers, hospitals, healthcare centers, clinics, staff quarters, support facilities, parking lots, sidewalks, water distribution systems, and sanitation facilities.

• Projects may include design-build (DB) and design-bid-build (DBB) services.

• Specific NAICS codes covered include Residential Remodelers, Industrial Building Construction, Commercial and Institutional Building Construction, Water and Sewer Line and Related Structures Construction, Highway, Street, and Bridge Construction, and Site Preparation Contractors.

• Typical required services also include buildings, systems and equipment, roofs, horizontal construction/repair, utilities, and dismantling/demolition.

• Design services for design-build task orders will require qualified A-E multi-discipline firms.

• Contract Type: Indefinite Delivery/Indefinite Quantity (IDIQ) Multiple-Award Task Order Contract (MATOC).

• Period of Performance: Base period of one year from award, plus four one-year optional periods (with a possible 1-6 month extension), for a total of up to five years and six months.

• Seed Project Period of Performance: 365 calendar days after receipt of Notice to Proceed.

• Minimum Guarantee: $1,000 per awardee.

• Maximum Task Order Amount: $5,000,000.

This information will be used as part of Volume I – Factor 3 – Past Performance in the proposal evaluation.

Contractor Information: Requires firm name, CAGE code, address, SAM UEI, contact details, and details about the work performed (prime, subcontractor, joint venture), including the percentage of work and prime contractor information if applicable. Contract Information: Requires contract number, task order number, contract type, title, location, award and completion dates, original and final contract prices, and a description of the project, including its complexity and relevance to the current solicitation. Client Information: Requires client name, title, contact details, and their role in the project. Performance Ratings: Clients are asked to rate the contractor's performance using adjective ratings (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable) across several categories: Quality, Schedule/Timeliness, Customer Satisfaction, Management/Personnel/Labor, Cost/Financial Management, Safety/Security, and General. Remarks: A section for clients to provide additional narrative comments, specific strengths, weaknesses, deficiencies, or overall ratings.

Bidders must ensure this questionnaire is completed by their clients for relevant past contracts. The completed questionnaire is a critical component of the proposal submission for the Past Performance factor. The instructions note that the client should submit the questionnaire directly to the offeror, who will then submit it with their proposal. The government reserves the right to verify all information provided.

Statement of Work (SOW)

This Statement of Work (SOW) outlines the replacement and upgrading of various architectural and infrastructure issues at the Kyle Health Center in Kyle, South Dakota. The goal is to modernize components to comply with current healthcare standards, enhancing patient care and satisfaction, and improving staff engagement.

• A one-year warranty covering material, labor, travel, and expenses is required.
• All demolished materials must be disposed of in compliance with relevant regulations.
• The contractor is responsible for providing temporary utilities.
• Compliance with tribal employment ordinances and obtaining a business license from the Oglala Sioux Tribe is required for work on the Pine Ridge Indian Reservation.
• The contractor is responsible for site security, including tools and equipment.
• The contractor must comply with Infection Control Risk Assessment (ICRA) and Pre-Construction Risk Assessment (PCRA) requirements.
• Submittals for materials, infectious control plan, safety plan, PCRA, quality control plan, Gantt chart, and schedule of values are required.
• A detailed schedule in Gantt chart form must be submitted within 14 calendar days of the Notice to Proceed.
• Progress meetings will be held at regular intervals, at least every other week.
• The contractor must ensure the site is clean of all debris and rubbish upon project completion and restore any property outside construction limits to its original condition.
