Statement of Work (SOW) / Performance Work Statement (PWS)

To provide a comprehensive maintenance service and support agreement for various GE Imaging equipment located at multiple facilities within the Great Plains Area Indian Health Service region (SD, ND, IA, NE). The contract will be a firm-fixed price type.

• Calibration/Preventive Maintenance: Includes all parts, labor, tools, equipment, literature, transportation, and per diem for onsite calibration, preventive maintenance, and mechanical/electrical safety testing as recommended by the OEM and required by accrediting bodies. Only new or OEM-certified replacement parts are to be used. A schedule of future/scheduled services must be submitted. Services must adhere to CMS, TJC, DNV-GL, and other accrediting entity windows (Quarterly: +/- 10 days, Semi-annual: +/- 20 days, Annual: +/- 30 days).

• Unscheduled Maintenance and Parts: Covers all unscheduled maintenance, repair, emergency call back, recall, modification, and breakdown repair at no additional cost to the Government. Includes all parts, labor, tools, equipment, literature, transportation, and per diem. Only new or OEM-certified replacement parts are to be used.

• Ultrasound Probes: Includes probe repair/replacement coverage for a maximum of two (2) General/Specialty ultrasound probes per 12-month period. Loaner probes of the same make and model must be provided at no cost during servicing.

• Removal: Contractor shall not remove any device, operation option, or accessory without express written consent from the Contracting Officer Representative (COR) or Clinical Engineering.

• The period of performance is not explicitly stated but implied to be a contract term. Work is to be performed at multiple facilities within the Great Plains Area Indian Health Service region.

• Hours of Performance: Unless otherwise stated or negotiated, work shall be performed Monday through Friday, 8 am to 5 pm, excluding weekends and holidays.

• Equipment: A list of existing equipment is provided, with some items marked as TBD (to be determined) indicating medical equipment or accessories procured but not yet installed.

• Documentation: Contractor shall provide all inspection forms and labels. Preventive maintenance must include a checklist showing OEM recommended checks, performance, results, date next due, and recommended intervals. Service tickets/calibration records with the P.O. number must be presented for each service performed. One copy of the service ticket and PM checklist to be left with the Department Supervisor, and one copy provided to the COR within one week of service. Service tickets may be submitted via email to abr_oehbiomed@ihs.gov.

• Quality Control: Government reserves the right to review and verify work. Contractor must have a prior successful history.

• Basis of Payment: Payments are due upon receipt of complete and accurate invoices in accordance with the Prompt Payment Act. Invoices must contain all required information.

• Liability: Contractor is not released from liability for acts of omission, improper repair, negligence, or actions jeopardizing health and safety.

• Releases: No information generated under this contract may be released without Contracting Officer concurrence.

• Warranty: All contractor labor is warranted for 30 days. All parts and material are warranted for a minimum of 30 days. Warranty callback service is provided during the warranty period at no additional charge.

• Contractor's Terms & Conditions: Incorporated by reference; Federal Acquisition Regulations and other Federal Regulations, Laws, and Rules take precedence in case of conflict.

• Contracting Officer's Authority: Only the CO has the authority to bind the Government. No other employee or representative can modify the contract terms or scope.

• Damage caused by failure to operate equipment per manufacturer's instructions, abuse, misuse, neglect, or acts of nature (fire, lightning, flood, etc.).

• Operating supplies or accessories not furnished by the Contractor during normal PM inspection and services.

• Work external to the equipment or maintenance of accessories, alterations, attachments, or other devices not specifically covered.

• Equipment modified, altered, added to, improperly repaired, installed, or used in violation of manufacturer's instructions, or damaged from other than ordinary use.

It requires offerors to self-certify their eligibility as an "Indian Firm" with "Indian Ownership" in accordance with the Buy Indian Act. This certification is a requirement for responding to the solicitation and for contract award.

Sources Sought Notice / Market Research

This notice is for informational and planning purposes to conduct market research to determine the availability of Indian Economic Enterprises (IEE) for a potential Firm-Fixed Price, Non-Personal Service Type Contract. The contract would provide comprehensive maintenance service and support for various GE Imaging equipment located at multiple facilities within the Great Plains Area Indian Health Service (GPAIHS) region.

• Comprehensive maintenance service and support for GE Imaging equipment.

• The anticipated NAICS code is 811210 – Electronic Precision Equipment Repair and Maintenance.

• Type of Order: Firm-Fixed Price Purchase Order.

• Anticipated Period of Performance: Base Year, plus four (4) option years.

Service units are located at various facilities within the GPAIHS region, including specific locations in South Dakota and North Dakota.

• This is not a solicitation, and no contract will be awarded based on responses.
