Statement of Work (SOW) / Performance Work Statement (PWS)

This Statement of Work (SOW) defines the requirements for individuals and companies assisting the 309th Maintenance Support Group (309 MXSG) with equipment installation, relocation, repair, or modification.

• The SOW applies to various equipment and control/monitoring systems used by the 309 MXSG.

• Examples of covered equipment include, but are not limited to: Air Compressors, HVAC units, CNC Machining Centers, Welders, Refrigeration Units, and various types of Grinders and Lathes.

• For each project, the 309 MXSG will provide a detailed SOW with specific requirements, specifications, drawings, and safety information.

• Contractors must submit quotes using a standardized format, demonstrating understanding through preliminary drawings, schematics, or technical data.

• Quotes must include a breakdown of costs, including materials, labor (with hourly rates), subcontractor costs, training, rentals, fees, overhead, and other direct costs.

• The 309 MXSG will evaluate quotes based on cost and how well they address requirements.

• Contractors must provide status reports as requested and a final project report upon completion, which may include fabrication drawings, source code, operation/maintenance instructions, and recommended spare parts lists.

• Onsite training for operators and maintenance personnel may be required.

• Contract Data Requirements Lists (CDRLs) are specified for Progress Reports (A001), Final Project Reports (A002), Contractor Safety Plans (A003), and Accident/Incident reports (A004).

• Place of performance is Hill Air Force Base (AFB), UT.

• Specific periods of performance will be defined at the call level for each action.

• Security: Requires compliance with various security protocols, including obtaining Defense Biometric Identification System (DBIDS) cards and potentially Controlled Area Badges (CABs). Personnel must adhere to Operations Security (OPSEC) and Communications Security (COMSEC) requirements. Introduction of personal electronic devices is strictly controlled.

• Safety: Contractors must implement a safety program plan, comply with OSHA and Air Force Occupational Safety and Health Standards, and ensure personnel are trained and qualified. Mishap reporting procedures are detailed.

• Quality Control: Contractors must maintain an effective quality control program.

• Government Provided Support: Government will provide working space, heat, light, ventilation, and electrical outlets. Government telephones are for official use only.

• Manpower Reporting: Contractors must report all labor hours via a secure data collection site (http://www.SAM.gov).

• Conflicts of Interest: Strict rules apply regarding conflicts of interest, especially for personnel employed by the Department of the Air Force.

• Identification: Contractors must clearly identify themselves as such at all times using specific protocols for badges, email, and verbal communication.

• Environmental: Awareness of hazardous materials and compliance with environmental laws and regulations is required. Utility conservation practices must be followed.

• Hours of Operation: Normal hours are Monday-Friday, 0530-1600, excluding Federal Holidays and base closures. Contractors are not expected to work on Down Days/Weather Days.

This document outlines the procedures for contractor credentialing required for the 309 MXSG IPE BPA (Indefinite Delivery/Indefinite Quantity - Broad Area Agreement) projects. It details the requirements for obtaining access badges (DBIDS) and other necessary credentials for contractor personnel working on these projects.

• Credentialing Authority: 309 MXSG/EN determines the need for contractor credentialing.
• Eligibility: Primarily for personnel directly supporting IPE BPA projects; vendors, salespeople, and delivery drivers are generally not approved unless associated with a specific task order.
• Access Badges (DBIDS): Issued for the length of the task order, not as "paper-passes." Temporary badges may be required for specific work areas.
• Scope Limitations: Contractors must not enter areas or perform work outside the scope of the IPE BPA or issued task order.
• Credential Issuance: Number and duration are case-by-case. Initial issuance is typically 5-7 DBIDS. Additional credentials require justification and coordination with MXSG/EN and MXSG Security.
• Project Duration: Projects 61+ days can request DBIDS via Form 496 for the task order length. Projects <60 days are sponsored via Southgate Visitor Center.
• Form 496: Must be returned to Bldg. 843, Room J106. Failure to return results in no additional credentials.
• Required Documentation: A copy of the requestor's "Real ID" Driver’s License is required with DBIDS requests.
• Employee Changes: Contractors must notify MXSG EN within 1 business day of employee separation and return terminated employee credentials within 1 business day. New credentials are not issued without receipt of terminated credentials.
• Driving Requirements: Driving through controlled movement areas requires additional training and an airfield driver’s license, if designated in the BPA or task order.
• Training: All required training must be completed before credential issuance. Yearly training is provided by MXSG Security and must be coordinated before existing badge expiration. Failure to complete training results in credential revocation.

Potential bidders must understand these credentialing procedures to ensure their personnel can access necessary work areas and comply with base access requirements. Failure to adhere to these procedures can result in removal from the contract or installation. Refer to the IPE BPA SOW for further details.
