This document is Attachment 2, the Contractor Information Form, designed to collect general information about potential vendors. It is intended to help the government understand a company's capabilities, business type, and relevant experience for potential teaming opportunities or contract awards.

• General Information: Business Name, Address, Point of Contact, and whether the company wishes to be publicized for teaming opportunities.
• Type of Business: Classifications such as Small Business (including 8(a), HUBZone, Women-Owned, Service-Disabled Veteran-Owned, Small Disadvantaged), NAICS Codes, and whether the firm acts as a Prime Contractor or Subcontractor.
• Bonding Capacity: Information on maximum bonding capacity per project and aggregate capacity.
• Acquisition Interest: Interest in performing Classified Projects.
• Contractor Arrangement: Details on participation in Mentor Protégé Programs or joint ventures.
• Capability: Information on accounting systems, Higher-Level Contract Quality certifications, and security clearances (Top Secret, Secret).
• Security: Details on SCIF ownership, NIST SP 800-171 compliance, experience with DISS, Scattered Castle, Lenel OnGuard, key control, access control procedures, courier authorizations, entry/exit inspections, and PII protection.
• Operational Readiness: Information on staffing contingency plans, AED training, and driver's license verification for personnel operating government vehicles.

Bidders must complete this form accurately and thoroughly to provide the government with essential details about their organization. The information collected will be used to assess suitability, capabilities, and eligibility for the opportunity, and may influence teaming decisions or contract awards. Specific sections on security clearances, certifications, and experience are critical for opportunities involving sensitive information or specific operational requirements.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide unarmed Access Control Officers and Escort personnel support services for HQ INSCOM and its subordinate commands within the United States. The services aim to control and monitor access of personnel and equipment, ensure personnel have required clearances, and provide escorts for individuals without security clearances.

• Control access of personnel and equipment into and out of facilities.

• Utilize Government-provided security equipment, systems, and databases.

• Conduct identification checks for building access.

• Conduct entry-exit inspections and form validation for all items.

• Provide escort services for non-cleared personnel.

• Document security-related activities and results.

• Performance will be evaluated based on the Government's Quality Assurance Surveillance Plan (QASP).

• Key performance objectives include Staff Management (95% minimum), Access Control operations (100% no unauthorized access, 95% minimum for equipment/media control), Facility and Personnel Security Services (95% minimum), and Escorts (95% minimum).

• Remedies for performance below threshold include invoice reduction.

• Ordering Period: One 12-month Base Year with four available 12-month option years.

• Period of Performance: To be identified at the Task Order level.

• Place of Performance: Aberdeen Proving Ground, MD; Fort Meade, MD; Charlottesville, VA; Alexandria, VA; Fort Belvoir, VA; Kileen, TX.

• Contractor must possess and maintain a Top Secret (TS) facility clearance at contract award and for the life of the contract.

• Contractor personnel must be U.S. Citizens with active TS/SCI security clearances.

• Specific personnel qualifications are detailed for Access Control Officers and Escorts, including age, education, experience, and certifications (CPR, AED).

• Comprehensive training requirements are outlined, including Information Assurance, Intelligence Oversight, iWatch, OPSEC, and AT/FP training.

• Strict security requirements include handling of classified information, CUI, PII, and adherence to various security directives and regulations.

• Contractor must develop and maintain a Quality Control Plan (QCP) and a Staff Management Plan.

• Transition-in and transition-out plans are required, with specific timelines and procedures.

• Contractor must comply with all applicable laws and regulations of the host country and U.S. Army regulations.
