This document is a Cybersecurity Supply Chain Risk Management (C-SCRM) Questionnaire that vendors must complete as part of their offer submission. It collects information regarding the vendor's practices and policies related to C-SCRM.

• Contact Information: Requires vendor company name, primary Point-of-Contact (POC) details (name, title, phone, email).
• Vendor Risk Management Plan: Asks about the vendor's identification of supply chain threats, mapping of key suppliers to these threats, and the existence of written SCRM requirements in contracts with suppliers. It also inquires about verification of supplier compliance.
• Physical and Personnel Security: Inquires about policies for employee background checks, procedures to prevent tampering of Information and Communications Technology (ICT) equipment in inventory, and insider threat awareness training.

Vendors must provide responses in the designated "Vendor Response" column for all applicable items. The government may request documentation to validate these responses. Completion of this questionnaire is a mandatory step for offerors.

Solicitation / RFP / RFQ

This Request for Quotations (RFQ) seeks proposals for internet service for the U.S. Embassy in Bangkok, Thailand. The government intends to award a contract for this service based on a comparative evaluation of offers, aiming for the best value.

• Provision of premium quality internet service with guaranteed speed (500/500 MBPS International/Domestic).

• Installation at 120-122 Wireless Rd, Lumpini, Pathumwan, Bangkok 10330.

• Minimum of four public internet routable addresses with a /28 subnet mask, not filtered or blocked for any IPSEC ports or protocols; no Network Address Translation (NAT).

• Packet Loss <= 0.5% and Availability of 99.9%.

• Dedicated and unlimited bandwidth, not shared or bundled.

• Support for all IP protocols without filtering, firewalls, proxies, or stateful inspections.

• Ability to ping 169.252.0.0/16 and 169.253.0.0/16.

• Physical interface: Gigabit, LC fiber connection, multimode fiber.

• Support for installation of customer-supplied VPN encryption device.

• Wi-Fi must be disabled on the router.

• 24/7 technical support.

• Contractor must not be a reseller, broker, or third-party.

• Minimum 7 years of experience in the ISP industry.

• Fiber Optic connectivity to the U.S. Embassy Bangkok compound.

• No ZTE, Huawei, Xiaomi, or affiliated branded products in use on the internet connection.

• The contract will include a one-year base period (9 months for the first year) and three one-year option periods.

• The contract type is Firm Fixed Price (FFP).

• Prices are to be quoted in Thai Baht.

• Value Added Tax (VAT) is not applicable due to the Embassy's tax exemption.

• Quotations are due by January 30, 2026, at 16:00 hrs (Bangkok local time).

• Questions must be submitted in writing by January 16, 2026, at 16:00 hrs (Bangkok local time).

• Submissions must be in English and in Adobe Acrobat (pdf) format, not exceeding 25MB per file.

• Offers will be evaluated using FAR 13 Comparative Evaluation, considering technical, past performance, and price.

• This is NOT a lowest price technically acceptable (LPTA) or trade-off process.

• The acquisition is UNRESTRICTED.

• Offerors must be registered in the System for Award Management (SAM) database prior to proposal submission.

• Required forms include SF-1449, Section 1 (Pricing), Section 5 (Representations and Certifications), CYBERSECURITY SUPPLY CHAIN RISK MANAGEMENT (C-SCRM) Questionnaire, and State Department Secure Software Development Attestation Form.
