- How the company will receive at least 51% of the earnings from the contract.
- Details on partnerships and subcontractor earnings (if applicable).
- How management and daily operations will be controlled by one or more individuals who are Indians.
- Identification of the company's size status (e.g., small business, HUBZone, SDB, SDVOSB, 8(a), VOSB, WOSB).
- Inclusion of the Buy Indian Act Indian Economic Enterprise Representation Form.
