Sources Sought Notice

This notice is issued by the Indian Health Service (IHS) to identify potential Indian Small Business Economic Enterprises (ISBEEs) and Indian Economic Enterprises (IEE) capable of performing a requirement for physical therapy services at the Crow Service Unit, Crow Agency, Montana. This is NOT a solicitation, but an RFI to gauge market interest and capability. The IHS intends to use the Buy Indian Act authority to give preference to IEEs where authorized and practicable.

• Period of Performance: One (1) Year with Four (4) Option Years
• Work Schedule: Weekdays, Monday through Friday, 8:00 AM to 4:30 PM.
• Services: Provide physical therapy services at the Crow Service Unit.
• Unique Requirement: The Crow / Northern Cheyenne Hospital serves the Crow Indian Reservation and requires continuity of care, ideally with the same physical therapist assigned each year.

Vendors are strongly encouraged to respond and must demonstrate their capability to perform the work. Responses should include, but are not limited to:

• Provider Availability & Qualification Verification: Describe the process for ensuring an active roster of qualified physical therapists, their credentials, and timely presentation. Provide current network size, breakdown by specialty, average timeframe for candidate presentation, and quality assurance processes.

• Provider Roster Transparency: Provide information on available physical therapists, including names (or number if names cannot be shared), geographic distribution, specialty certifications, credentials, training, current availability, and commitment.

• Service Delivery Commitment: Confirm ability to provide the same physical therapist for the specified schedule, note any limitations, and describe approaches to ensuring consistent on-site presence and backup coverage.

• Additional Information: Firm name, address, contact info, UEI number, Buy Indian Act IEE Representation Form, relevant past performance (especially with IHS or tribal programs), demonstration of receiving at least 51% of earnings, and how management/operations are controlled by Indians.

Failure to address the requested items may exclude a firm from being determined a capable IEE/ISBEE source. Responses will assist in determining the acquisition strategy.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for non-personal service Physical Therapy Services to support the mission of the Indian Health Service (IHS) at the Crow/Northern Cheyenne Indian Hospital and associated clinics in Montana.

• Provide professional physical therapy rehabilitative care for patients.

• Services include direct patient care, provider duties, and management of patient needs.

• The contractor will supply the same physical therapist for each year of the contract to ensure continuity of care.

• Tasks include providing a full range of rehabilitative physical therapy for all age ranges, exercising knowledge of PT procedures, correlating methods with various medical problems, consulting with patients and health professionals, conducting therapeutic activities, and applying standardized and innovative approaches.

• All documentation must meet established IHS Medical Facility, Joint Commission, and CMS standards for timeliness, legibility, accuracy, content, date, time, signature, and designated profession.

• Performance will be evaluated based on a Performance-based Matrix, focusing on Competency, Compliance, Quality, Patient Outcomes, Professionalism, and Documentation.

• Substantiated reports of breaches in patient safety, infection control, or other adverse patient outcomes constitute a breach of contract.

• Place of Performance: Crow/Northern Cheyenne Indian Hospital, with potential services at Lodge Grass Clinic and Pryor Clinic, Montana.

• Period of Performance: A base year of 12 months, with four (4) one-year option periods. (Base Year: March 1, 2026 - February 28, 2027; Option Years extend through February 28, 2031).

• Contractor Physical Therapists must possess at least 10 years of physical therapy experience, including 2 years in outpatient, inpatient, swing bed, and home health.

• Must hold a current, valid, unrestricted physical therapist license.

• Must possess a Basic Life Support (BLS) certification.

• Must have passed the National Physical Therapy Examination (NPTE).

• Must provide documentation for various medical evaluations and immunizations (Hepatitis A/B, MMR, Varicella, Tdap/Td, Meningococcal, TB test, Influenza - mandatory, COVID - highly encouraged).

• Background checks and security investigations are required.

• Contractor must adhere to HIPAA, Privacy Act, and IHS Service Unit/Health Center specific policies.

• Contractor must wear a government-issued contractor identification badge.

• Contractor must attend mandatory orientations and training.

• Must read, understand, speak, and write English and be respectful of American Indian and Alaska Native culture.

• Maintain professional demeanor and good communication.

• Deliverables include a Monthly Data Report.

• Government will provide facility-specific contractor identification badges (fee for lost badges).

• Government will provide necessary equipment, supplies, and clinic space, except for specific contractor-furnished items like uniforms and personal medical instruments.

This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Period: Offerors must meet the definition of an "Indian Economic Enterprise" at the time of offer, contract award, and throughout the contract performance period. Any change in eligibility must be immediately reported.
• Representation: Offerors must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Information: The form requires the Offeror to provide:
