This document is the VA Notice of Limitations on Subcontracting—Certificate of Compliance for Services and Construction. It requires offerors to certify their compliance with subcontracting limitations as stipulated in the solicitation and resultant contract, pursuant to 38 U.S.C. 8127(k)(2).

• Services Contract: The contractor must certify that no more than 50% of the contract amount will be paid to firms that are not VIP-listed Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) or Veteran-Owned Small Businesses (VOSBs). Work further subcontracted by a similarly situated VIP-listed subcontractor counts towards this limit. Other direct costs may be excluded under specific conditions.
• General Construction Contract: The contractor must certify that no more than 85% of the contract amount will be paid to firms that are not VIP-listed SDVOSBs or VOSBs. Cost of materials are excluded.
• Special Trade Construction Contractors: The contractor must certify that no more than 75% of the contract amount will be paid to firms that are not VIP-listed SDVOSBs or VOSBs. Cost of materials are excluded.
• Certification Acknowledgment: Offerors acknowledge that false certifications are subject to penalties under Title 18, United States Code, Section 1001.
• Compliance Verification: The VA may require documentation to verify compliance during contract performance and upon completion.
• Required Fields: The certification section requires the printed name and title of the signee, signature, date, and company name and address.

This is a mandatory certification that must be completed, signed, and returned with the offeror's bid, quotation, or proposal. Failure to provide this completed certification will render the offer ineligible for evaluation and award. Bidders must understand and agree to the subcontracting limitations applicable to the contract type (services, general construction, or special trade construction) to ensure compliance if awarded the contract.

This document is a Price Schedule for the base year of a contract, detailing specific services and their associated costs. Bidders are expected to use this schedule to provide their pricing for each CLIN (Contract Line Item Number).

• CLIN 0001: Repair of Kitchen Hood Deficiencies at NFS, Canteen, and CLC locations.
• CLIN 0002: Labor and equipment to close Annual deficiency findings, including battery replacement and sensor strobe equipment, in Parking Garage, Buildings 5, 6, 33, and 37.
• CLIN 0003: Labor and equipment to close Quarterly Sprinkler deficiency findings in Buildings 1, 5, 6, 7, 8, 12, 13, 20, 22, 33, 34, 36, and 45.
• Period of Performance (POP): Base Year is June 1, 2026 – May 31, 2027 for all CLINs.
• Units of Measure (U/I): JB (Job) for all CLINs.
• Pricing Fields: Bidders must provide Unit Price and Total Amount for each CLIN.

This price schedule is critical for bidders to understand the specific services required and to formulate their pricing proposals. Bidders must accurately complete the Unit Price and Total Amount fields for each CLIN to submit a compliant bid.

Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure all buildings and property at the Western Health Care System (WCHCS) Medical Center in Grand Junction, CO, meet fire code safety requirements through repair, inspection, testing, and preventative/corrective maintenance of fire alarm and sprinkler systems.

• Corrective Maintenance & Documentation: Address repairs indicated in deficiency reports for fire alarm, sprinkler, and kitchen hood systems. This includes visual notification, electrical, plumbing, HVAC, and mechanical sub-systems.

• Inspections, Testing, and Maintenance: Perform all required inspections, testing, and preventative/corrective maintenance to enhance system reliability and prevent downtime.

• Specific Tasks by CLIN:

  • CLIN 0001: Repair Kitchen Hood Deficiencies (NFS, Canteen, CLC locations), including manifold repair, nozzle replacement, CO2 cartridge replacement, and hydro testing.
  • CLIN 0002: Annual Service Deficiency Repairs, including battery replacement and sensor strobe equipment for fire alarm systems in various buildings and the parking garage.
  • CLIN 0003: Quarterly Sprinkler Deficiency Repairs, including 5-year internal inspections, valve replacements, gauge replacements, and signage installation across multiple buildings.

• Compliance: All work must comply with the latest editions of NFPA standards (NFPA 4, 10, 13, 25, 70, 72, 75, 80, 92, 96, 101, 204), Joint Commission standards, OSHA, and manufacturer requirements. The more stringent requirement applies in case of conflict.

• Reporting: Provide daily field service reports to the COR. Within 10 business days of completion, submit a detailed written service report including test results, parts replaced, identified deficiencies, and status of corrections. Reports must be broken out by EP with specific details on NFPA codes, devices tested, locations, and recorded values.

• Deficiency Proposals: If deficiencies cannot be corrected on-site, provide a proposal with a timeline for repair.

• System Integrity: Maintain operational integrity of fire alarm systems during testing. Notify COR 3 weeks in advance for work expected to interrupt operations or require utility outages.

• Period: One base year. Performance outside normal hours (Mon-Fri, 7:00 am - 4:30 pm MST) requires 14 days advance notice to the COR.

• Place: Western Colorado Health Care System, 2121 North Ave, Grand Junction, CO 81501. Work to be performed in various specified buildings.

• Personnel Qualifications: Field Service Engineers must be factory trained and certified by the original manufacturer. On-site supervisory employee requires NICET-FAS Level 3 certification; Fire Alarm Technicians require NICET-Level 1 certification.

• Parts: All replacement parts must be Original Equipment Manufacturer (OEM).

• Security: Contractors must comply with VA Directive 6500 regarding information security. Escorts required in patient-sensitive and IT spaces. No copies of patient information allowed.

• Contract Type: Firm Fixed Price.

• Post-Award: A contract kickoff/post-award meeting is required before performance begins, unless waived by the CO.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for fire system inspection and repair services for the VA Western Colorado Healthcare System located at 2121 North Ave., Grand Junction, Colorado 81501. This announcement constitutes the only solicitation; no separate solicitation document will be issued. The government anticipates awarding a firm-fixed-price contract.

• Fire System Repair Services for the VA Western Colorado Healthcare System.

• Services include inspection and repairs.

• Offerors must be an Original Equipment Manufacturer (OEM) or authorized dealer with OEM equipment certification for services and maintenance contracts. No remanufactured or gray market items are acceptable.

• NICET certifications are required: Valid NICET-FAS Level 3 for the on-site supervisor and valid NICET-Level 1 for anticipated technician(s).

• Contract Type: Firm-fixed-price.

• Period of Performance: One year only (specific dates not detailed in this section, but implied by response date).

• Option to Extend Services: The government may require continued performance for up to 6 months, with rates adjustable based on prevailing labor rates.

• Response Date/Time: May 7, 2026, 5:00 PM Mountain Time, Denver, USA.

• The government intends to award based on initial quotes received without discussions.

• Award will be made on an All or Nothing Basis.

• Set-Aside: SDVOSB (Service-Disabled Veteran-Owned Small Business) total set-aside. Quotes will only be accepted from SDVOSBs.

• SBA Verification: SDVOSB status must be SBA verified and visible in the Small Business Search Certification at the time of quote submission and award.

• NAICS Code: 561621 (Security Systems Services, except Locksmiths) with a business size standard of $25 Million.
