This Statement of Work (SOW) outlines the requirements for construction services at the VA Northern California Healthcare System (VANCHCS), Mather VAMC, for Project 612A4-26-050: Install B644 Dermatology Generator Site Prep. The project involves furnishing all labor, materials, equipment, supervision, and professional engineering services for the construction effort.

• Part 1: Generator Enclosure Area: Construct a permanent, approximately 500 sq ft mobile generator enclosure. This includes demolition of existing asphalt, concrete, and landscaping; subgrade preparation and compaction; installation of new asphalt pavement; and erection of an 8-foot-tall galvanized steel chain-link fence with privacy slats and specific gate configurations (two single-leaf personnel gates, one double-leaf generator access gate).

• Part 2: Sidewalk Around Generator Enclosure: Modify existing sidewalks to create a safe pedestrian path around the new enclosure. This involves selective demolition and construction of two ADA-compliant concrete transition ramps and establishing a designated, painted pedestrian walkway.

• Part 3: Electrical Work: Relocate the existing generator tap box approximately 35 feet to the new enclosure location. This includes extending existing conduit, installing new conduit through a masonry wall, replacing conductors as needed, and ensuring the relocated tap box is properly mounted and tested.

• Part 4: Sidewalk Corrections Around Building 651: Address sidewalk deficiencies at the northeast corner of Building 651. This includes removing unpaved areas, constructing new concrete pavements, extending an accessible ramp, and ensuring ADA compliance and proper drainage. It also involves relocating a STOP sign and NO RIGHT TURN sign, applying a "YIELD" asphalt marking, and establishing a temporary ADA-compliant pedestrian walkway during construction.

• Part 5: Artificial Grass Installations: Provide and install approximately 8,000 SF of artificial grass system, including site prep and drainage, according to manufacturer requirements and approved shop drawings.

• Part 6: Handrail Installation: Furnish and install approximately 75 linear feet of permanent stainless-steel handrail along the sidewalk north of Building 644, adjacent to an artificial grass area. Includes a continuous concrete handrail curb and compliance with VA specifications and ADA requirements.

• Location: Mather VA Hospital – 10535 Hospital Way Bldg. 700, Mather, CA, 95655.

• Completion Time: Maximum of 180 days from commencement.

• Scheduling: Submit a comprehensive work schedule; updated schedules with each payment application; construction during normal working hours with limited off-hours premium time for specific reasons.

• Site Visits: By appointment only with the Medical Center COR.

• Testing: All testing by certified laboratories subcontracted by the Contractor.

• Security: All employees must comply with VA security management program, obtain VA police permission, be identified, and restricted from unauthorized access. Specific security and records management requirements are detailed, including handling of sensitive information and data.

• Submittals: Utilize an electronic submittal exchange project website.

• Competent Person: OSHA designated "competent person" must be present at the work site whenever workers are present.

• Pricing Proposal: Must include breakdown pricing for the General Contractor and all subcontractors.

• Compliance: Adherence to VA Barrier Free Design Standard, ABBAS, NEC, and other applicable VA specifications.

• Waste Management: Contractor to provide dumpsters for debris disposal.

• Subcontractor Approval: Contractor requires Contracting Officer approval for any subcontracts involving disclosure of project information.
